• All pharmaceutical products must include bar code labeling at the unit-of-use package level, conforming to GS1-128 or HIBCC standards.

• Only products with a therapeutic equivalence code of "A" by the FDA will be considered, unless all drugs in a family group are "B" rated.

• Offerors must provide an eleven-digit National Drug Code (NDC) for each product.

• Package sizes below 500 count require a Safety/Child Proof cap.

• Expiration dates must be no less than 12 months from the date of delivery.

• Substitutions of pack sizes, deviations from specific items, partial deliveries, and more than three (3) lot numbers require written approval from the Contracting Officer.

• Minimum bottle size is 100cc with a child safety type cap.

• Shipments must include a packing slip with specific details (Vendor name/address, PO Number, Obligation Number, NDC, products, line-item numbers, vendor contact for discrepancies).

• Compliance with the Drug Supply Chain Security Act (DSCSA) is required, including submission of Transaction History (TH), Transaction Information (TI), and Transaction Statement (TS).

• Delivery is FOB Destination to VA CMOP TUCSON.
