This is a combined synopsis/solicitation (RFQ) for Radiological Technologist/Laboratory Assistant Services for the Jicarilla Service Unit in Dulce, NM. • Provide one (1) Radiological Technologist/Laboratory Assistant.

• Health requirements include specific vaccinations (Measles, Mumps, Rubella, Hepatitis B, Varicella, Influenza, Pertussis, COVID-19) or documented immunity/contraindications, and a TB test within the last 12 months.

• Contract Type: Firm-Fixed-Price (FFP) Purchase Order.

• Period of Performance: One (1) Base Year (TBD 2026-TBD 2027) with four (4) one-year Option Periods (TBD 2027-TBD 2031).

• Total contract period not to exceed five years.

• Place of Performance: Jicarilla Service Unit, 500 North Mundo Road, Dulce, NM 87528.

• Government-furnished items include necessary equipment, supplies, clinic space, and PPE (excluding uniforms/lab coats).

• Contractor-furnished items include uniforms, lab coats, and other personal medical instruments.

• Food and lodging are the responsibility of the contractor.

• Quotes are due by 05/13/2026 at 1730 MT.

• Quotes must be submitted electronically to the Contracting Officer.

• Submission Requirements: Quotes must be submitted in three volumes: Technical Capability & Staffing Approach, Past Performance, and Price.

• Evaluation Factors: Technical Capability and Staffing Approach (most important), Past Performance, and Price. The Government will award based on the best value determination, which may involve a tradeoff.

• Offerors must complete and submit the SF-18 form (page 1 only) and the IHS Buy Indian Act Indian Economic Enterprise Representation Form.

• This acquisition is set-aside for small business concerns.

• First Tier: Indian Small Business Economic Enterprise (ISBEE).

• Second Tier: All Other Small Business Concerns.

• Offers from enterprises not meeting these criteria will not be considered.

• The solicitation constitutes the only solicitation; no separate written solicitation will be issued.

• The NAICS code is 561320 with a small business size standard of $34 million or less.

• Fingerprinting and background investigation are required prior to performance, which may take up to six months.

• Pricing must be an all-inclusive rate.

• Past performance information will be evaluated from the last five years.
