This document is Attachment 3, a Technical Certification Form, required to be completed, signed, and returned with a quote for Solicitation# SP4510-26-Q-1008. Failure to submit a completed form will render a submission ineligible for award.

• Certification (Yes/No) for each minimum required specification, including:
  • Diesel Engine (Tier III)
  • Minimum lifting capacity (10,000 lbs retracted, 6,000 lbs at 15 ft height, 4,000 lbs at 21.5 ft reach)
  • Load Chart compliance (ASME B56.6)
  • Ground clearance (>= 16.5 inches)
  • Fork tine length (72” +/- 2”)
  • Solid rubber tires
  • Forward reach (25’- 30’)
  • Frame leveling capability (8-10 degrees tilt)
  • Steering modes (2 Wheel, 4 Wheel, Crab)

This form directly dictates the technical specifications for the telehandler being procured. Bidders must carefully review each requirement, confirm their proposed equipment meets these specifications, and provide all requested business and technical documentation. Accurate completion and submission are mandatory for quote evaluation.

Statement of Work (SOW)

This Statement of Work (SOW) outlines the requirements for the acquisition of four (4) new 10K Telehandlers for the Defense Logistics Agency (DLA) Disposition Services (DS) in Kuwait.

• Procurement and delivery of four (4) new 10K Telehandlers.

• Equipment must meet specific salient characteristics, including a diesel engine (Tier III), a minimum lifting capacity of 10,000 lbs, and specific dimensions for fork tine length and ground clearance.

• The telehandlers must have an enclosed cab with heater, air conditioner, and windshield wipers.

• OEM parts supply must be available in the CENTCOM AOR.

• Equipment must be set up and operational within ten (10) business days after delivery.

• Contractor must provide at least ten (10) business days advance notice before rendering services at the site.

• Contractor is solely responsible for all spills or leaks during performance, including cleanup at no cost to the USG.

• Delivery is required within six (6) months of purchase order receipt.

• Delivery destination is DLA Disposition Services Kuwait, BLDG T-835, Zone 7, APO AE 09366.

• Delivery will occur during normal government working days (Monday-Friday), excluding USG holidays.

• Equipment must have a DoD item unique identification (IUID).

• Contractor must be present for delivery, unload, and assemble equipment.

• Contractor must complete and sign the DD250 Material Inspection and Receiving Report.

• Equipment must come with the manufacturer's standard warranty.

• Invoicing will be processed through the DoD Wide Area Workflow (WAWF) system.
