Statement of Work (SOW) / Performance Work Statement (PWS)

To provide residential housing and associated support services for Veterans in the Substance Use Disorder (SUD) Treatment Program administered by the Veterans Health Care System of the Ozarks (VHSO) and its Community-Based Outpatient Clinics (CBOCs).

• Provide residential beds, random drug testing, and other support services.

• Services to be provided at facilities within a 100-mile radius of VHSO campus in Fayetteville, AR, and CBOC campus in Springfield, MO.

• Offerings include equipment, materials, supplies, food, recreational supplies, internet/Wi-Fi, PCs/laptops/tablets, labor, training, supervision, and transportation.

• Contract duration is a 5-year Indefinite Delivery Contract (IDC).

• Facility requirements include adequate heating/cooling, working plumbing, and adherence to temperature logs (65-80 degrees).

• Separate sleeping quarters for male and female Veterans.

• Availability of indoor/outdoor lounge and recreational areas.

• ADA compliant bathrooms and bedrooms.

• Specific room standards including clean mattresses, new pillows, adequate bedding, and secure containers for personal belongings.

• Common areas must have non-slip floors, stable shower seats, handrails, and clean/functional furnishings.

• Dietary requirements include nutritious meals, drinks, and snacks, with specific food group inclusions.

• Laundry services to accommodate 18 Veterans with operable washing machines and dryers, including laundry soap and dryer sheets.

• Specific tasks include compliance with VA security directives, employee training, safety/bed checks, and emergency protocols.

• Therapeutic and rehabilitative services must be provided, including recovery meetings, healthy activities, and personal hygiene assistance.

• Medication administration supervision by qualified staff (RN/LPN/CNA).

• Regular drug screens for staff and Veterans.

• Resident conduct and behavior standards including hygiene, dress code, respect, and facility rules.

• Residential activities include weekend events and recreational options.

• Medical needs management for Veterans, including secure medication storage and assessment by RN/LPN.

• Emergency/risk management for safety concerns and infestations.

• Inspections by COR/back-up COR and reporting of findings.

• Contract period: 5-year IDC award.

• Place of Performance: Facilities within a 100-mile radius of VHSO campus (Fayetteville, AR) and CBOC campus (Springfield, MO).

• Contractor employees must meet federal, state, and local certification requirements (e.g., Certified Medical Technician, Level I Medication Aide, Licensed Administrator).

• Staff must receive training and regular supervision.

• Transportation between contractor facility and VHSO is required Monday-Friday.

• Specific bed availability requirements for Fayetteville, AR (14 primary, 6 transitional) and Springfield, MO (10 primary, 6 transitional).

• Compliance with Americans with Disabilities Act (ADA).

• Facility must provide PC/laptop/tablet with internet/Wi-Fi for Veterans.

• Quarterly deep cleaning of the facility is required.

• Staff must be active in a recovery process free from drug and alcohol abuse.

• Collaboration with the clinical treatment team for best clinical outcomes.

• Immediate action required for safety concerns, infestations, or facility failures.
