Statement of Work (SOW) / Performance Work Statement (PWS)

This Scope of Work (SOW) details construction contract requirements for VA Maine’s Presque Isle Community Based Outpatient Clinic (CBOC) located at 732 Main Street, Presque Isle, ME 04769. The primary goal is to address identified patient safety findings within the leased facility.

The contractor shall provide all necessary labor, materials, equipment, and supervision to perform the following tasks:

• Install remote door operators on patient bathrooms (Rooms 109 and 105). Doors must be operable via a handicap button from both inside and outside.
• Install ADA compliant handrails in corridors 113 and 139. Specific start and end points for handrails are detailed, with provisions for multiple runs if a continuous one is not feasible.
• Install a VA-provided oxygen sensor in Room 137, oriented to face the Pact Team Work Area for visibility of the digital readout.

Work must be completed within 90 calendar days from the issuance of the Notice to Proceed.

• All work must comply with current building codes, industry standards, and VA standards.
• Installation must be performed by certified installers from relevant regulatory agencies and the manufacturer.
• Adherence to federal regulations and VA requirements for construction projects is mandatory, including OSHA certifications (30HR for site superintendent, 10HR for sub-contractors).
• Work is to be performed during normal business hours (7:00 am – 4:00 pm, Monday-Friday), with exceptions requiring prior approval from the VA contracting officer.
• The contractor must comply with federal and agency records management policies, including safeguarding Privacy Act information and obtaining approval before sub-contracting or disclosing contract-related information.
