Performance Work Statement (PWS)

To obtain testing, certification, repair, and replacement services for backflow prevention assemblies at five Veterans Affairs Palo Alto Health Care System (VAPAHCS) locations.

• Provide management, supervision, labor, parts, materials, tools, and incidental services for backflow prevention assembly services.

• Services include testing, certifying, repairing, and replacing backflow assemblies to meet manufacturer specifications and applicable codes.

• Modifications require written consent from the Government’s Contracting Officer’s Representative (COR).

• Work is required at five VAPAHCS locations: Palo Alto, Livermore, Menlo Park, Mountain View, and Sunnyvale.

• Contractor is responsible for all repair and replacement costs unless specified otherwise.

• Standard operating hours are 8:00 AM to 4:30 PM, Monday through Friday, excluding Federal Holidays.

• Successful testing or retesting of backflow preventers.

• Updated physical tags provided by the applicable city authority.

• Submission of certification records to the applicable city authority.

• Delivery of copies of certification records and service tickets to the COR.

• The contract covers services at five VAPAHCS locations listed in the document.

• Contractor must be licensed, bonded, and insured in California.

• Personnel must possess correct California Contractor license classifications and provide AWWA certified tester certificates.

• Contractor must have a minimum of five years of successful experience in testing, certifying, repairing, and replacing backflow assemblies.

• Offerors must submit references and proof of certifications.

• Contractor personnel require background checks and must wear VAPAHCS-issued identification badges.

• Contractor must establish a Quality Control Program within ten days of Notice to Proceed.

• Adherence to OSHA, EPA, NFPA Life Safety Codes, and other regulatory requirements is mandatory.

• Contractor must provide Safety Data Sheets (SDS) for hazardous materials and maintain Personal Protective Equipment (PPE).

Sources Sought Announcement

This is a Sources Sought announcement for market research regarding backflow maintenance and repair services for the VA Palo Alto Health Care System (VAPAHCS). The government is seeking information to make acquisition decisions and is not soliciting proposals or quotes at this time. No costs incurred in responding will be reimbursed.

The requirement is for a non-personal services contract to provide testing, certification, repair, and replacement services for backflow prevention assemblies at VAPAHCS. The contractor will be responsible for all labor, equipment, materials, tools, travel, and overhead necessary to perform these services.
