• Providing a Production Lot Quality Conformance Test Plan and conducting Quality Conformance Testing.

• OEM Information: If not the OEM, offerors must state the OEM (Company Name and CAGE) and provide a signed letter of authorization as a distributor.
• Packaging Facility: If different from offeror's address, provide Name, Street Address, and CAGE of the packaging facility.
• Quote Details: Include estimated repair price, cost of new material (actual or estimated), Teardown & Evaluation Rate, Repair Turn Around Time (RTAT), Throughput Constraint, and Induction Expiration Date.
• Exceptions: Specify any exceptions to MIL-STD Packaging, MIL-STD Labeling, Inspection & Acceptance locations, Packaging House, or surplus materials.
• Priority Rating: Any awarded contract will be DO certified under the Defense Priorities and Allocations System (DPAS).
