Submission Requirements

Offerors must provide:

• Repair Price: Estimated (EST) for the full repair effort, including costs for material receipt, complete repair (even for unwhole/damaged items), and CAV Reporting. Also include the cost of new (actual or estimated) for evaluation.
• Repair Details: Teardown & Evaluation Rate, Repair Turn Around Time (RTAT), Throughput Constraint, and Induction Expiration Date.
• Capacity Constraints: Reference proposed RTAT and any capacity constraints.
• Exceptions: Clearly specify any exceptions to MIL-STD Packaging, Labeling, Inspection & Acceptance locations, Packaging House, or use of surplus materials.
• Traceability: For surplus material, a certificate and label are required, with traceability back to a Government contract. Distributors/dealers must submit full traceability back to the OEM.
• Packaging Location: If different from the offeror's address, provide name, street address, and CAGE.
