Key Performance Requirements:

• Vendor shall not store, collect, process, or transmit any VA sensitive data. All system configuration data, activation logs, and preset configurations are VA-owned on a VA-hosted server.
• Remote implementation, configuration assistance, and technical support are required.
• Contractor personnel may remotely access the VA-hosted server only if VA grants access for implementation assistance, via VA-approved secure methods, and only for system configuration.
• Key personnel must be assigned, with specific procedures for substitutions.

Submission Requirements

Offerors must submit:

• The solicitation number, name, address, and telephone number of the Offeror.

• Information necessary to evaluate factors per FAR 52.212-2.
• Responses to provisions requiring Offeror completion of information, representations, and certifications (excluding SAM).
• A Performance Work Statement addressing the Statement of Objectives and performance standards.
• A list of products to meet requirements.

• A statement accepting or excepting to the terms and conditions.
• Mandatory Attachment: The 852.219-76 VA Notice of Limitations on Subcontracting - Supplies Certificate must be completed, signed, and returned with the offer. Failure to submit this form will render the offer ineligible.

Award will be based on a comparative evaluation of quotes, selecting the most favorable to the Government, considering:

• Technical: Extent to which products and service approach meet or exceed facility needs, including timeliness.
• Past Performance: Assessment of risk based on quoter's record of past performance.
• Price: Total evaluated price, including all line-item prices and options.
