Solicitation / RFP / RFQ

This solicitation is for the procurement of pharmaceutical products for the Department of Veterans Affairs (VA) Consolidated Mail Outpatient Pharmacy (CMOP) in Charleston, SC. The acquisition is unrestricted.

• Procurement of Verapamil HCl 240MG SA TAB (500CT), Verapamil HCl 120MG SA TAB (100CT), and Verapamil HCl 180MG SA TAB (500CT).

• Products must be shippable by USPS to the CMOP facility.

• All pharmaceutical products must include bar code labeling conforming to GS1-128 or HIBCC standards.

• Products must have a therapeutic equivalence code of "A" from the FDA, unless exceptions apply.

• Expiration dates must be no less than 12 months from the date of delivery.

• Products must meet specific packaging requirements, including child-proof caps for certain sizes and minimum bottle dimensions.

• Compliance with the Drug Supply Chain Security Act (DSCSA) is required for prescription pharmaceuticals, including submission of Transaction History (TH), Transaction Information (TI), and Transaction Statement (TS).
