Scope of Work
The successful offeror will provide Benazepril HCL Tablets in various strengths and package sizes, as detailed in the estimated annual requirements:
- Benazepril HCL 5MG Tablet: 100-count (2,563 bottles), 500-count (947 bottles)
- Benazepril HCL 10MG Tablet: 100-count (1,682 bottles), 500-count (4,502 bottles)
- Benazepril HCL 20MG Tablet: 100-count (2,208 bottles), 500-count (8,650 bottles)
- Benazepril HCL 40MG Tablet: 100-count (2,130 bottles), 500-count (7,893 bottles)
Offerors must state the exact drug name as it appears on the label and ensure the product has an NDC number unique to the offeror. The successful contractor's label with its unique NDC must be the only label on the product, and VA labels must be able to securely adhere on top of it.
Contract Details
- Contract Type: One contract will be awarded.
- Duration: One base year plus four pre-priced one-year option periods.
