• Original Due Date: November 26, 2025

• New Due Date: December 3, 2025, at 2:30 PM Central Time

• The due date and time for receipt of offers has been extended.

• The deadline for offerors to submit their proposals has been moved from November 26, 2025, to December 3, 2025, at 2:30 PM CST.

• Offerors are required to include a completed and signed copy of this amendment with their proposal.

• All other terms and conditions of the original solicitation remain unchanged.

Solicitation / RFP / RFQ

This solicitation is for the procurement of Levetiracetam Tablets in various strengths and package sizes. The contract will establish national contract prices for these pharmaceutical products, which will be distributed through the VA and Department of Defense (DoD) Pharmaceutical Prime Vendor (PPV) Programs.

• Procurement of Levetiracetam Tablets in the following strengths and package sizes:

  • 250mg: 120 tablets/bottle, 500 tablets/bottle
  • 500mg: 120 tablets/bottle, 500 tablets/bottle
  • 750mg: 120 tablets/bottle, 500 tablets/bottle
  • 1000mg: 60 tablets/bottle

• Unit of use bottles (120s) must meet specific safety cap and bottle volume/dimension requirements.

• Products must be FDA approved and manufactured in FDA cGMP acceptable facilities.

• Offerors must provide a unique 11-digit National Drug Code (NDC) for each offered product.

• Bar code labeling compliant with GS1-128 or HIBCC standards is required.

• Compliance with Drug Supply Chain Security Act (DSCSA) requirements is mandatory.

• Type: Firm Fixed Price, Indefinite-Delivery Requirements Contract.

• Period of Performance: Base year plus four one-year option periods.

• Distribution: Through VA and DoD Pharmaceutical Prime Vendor (PPV) Programs.

• Payment: Contractors will receive payments from PPV programs, not directly from the Government.

• Proposals must be submitted via email in Microsoft Word or PDF format, including a scanned copy of the signed SF-1449.

• Offerors must submit a price for all line items for the base year and all four option years.

• Prices must not exceed two decimal places and must include a 0.5% Cost Recovery Fee.

• Evaluation will be based on the lowest price technically acceptable offer.

• Key deadlines: Offer Due Date is November 26, 2025, 2:30 PM CT.

• This acquisition is unrestricted.

• Offerors not manufacturing the items must submit a Letter of Commitment from the manufacturer.

• Offerors of covered drugs must have a Master Agreement (MA) and Pharmaceutical Pricing Agreement (PPA) in place with the VA.

• Subcontracting plan requirements apply to other than small business concerns for contracts over $750,000.

• Estimated annual usage quantities for VA, DoD, BOP, and IHS are provided in Attachment C.

• A list of current VA and DoD PPV contractors is provided in Attachments A and B.
