Scope of Work
The successful offeror will supply Melatonin in 1mg, 3mg, and 5mg dosages. Key requirements include:
- Product Identification: Offerors must state the exact drug name as it appears on the label and provide a unique NDC/UPC number. The contractor's label must be the only label on the product, with VA labels adhering securely over it.
- Packaging: Unit of use packaging for all line items (1mg, 3mg, 5mg) must feature a safety-cap (deshellable/convertible) and a bottle with a minimum 120cc volume, cylindrical body, and minimum 5.75" circumference. Bottles must accommodate a 5.25" x 2" label, ensuring barcodes and expiration dates remain exposed.

Submission & Evaluation
Offerors are required to provide one sample (one bottle with proposed label attached) for each line item (1mg, 3mg, 5mg). Samples must be received by the due date and time for offers. Late samples may result in rejection of the offer. Samples will be evaluated for compliance with salient characteristics, the Scope of Contract, and Section E.8 (52.212-2 Evaluation). Only offers with compliant samples will be eligible for award. Samples should be sent to the address specified in the Schedule of Supplies and Addendum to FAR 52.212-1.
