• Manufacturer Certification: Offerors can certify they are the manufacturer or producer of the end item, manufactured in the United States.
• Legal Acknowledgment: Acknowledges that false certifications are subject to penalties under Title 18, United States Code, Section 1001.
• Consequences of Non-Compliance: Outlines potential penalties for awarded SDVOSB/VOSBs found not to have acted in good faith, including referral to suspension/debarment committees, fines, and prosecution.
• Record Keeping: Offerors may be required to provide documentation (invoices, subcontracts) to verify compliance.
• Certification Fields: Requires printed name, title, signature, date, and company information for the certifying individual.

This form is critical for bidders. It must be completed, signed, and returned with the offer. Failure to provide this completed certification will render the offer ineligible for evaluation and award. Bidders must carefully review and understand the subcontracting limitations and their own company's eligibility to certify compliance.

Solicitation / RFP / RFQ

This is a combined synopsis/solicitation for commercial products (Borescope Inspection Kits) for the Department of Veterans Affairs Eastern Colorado Healthcare System (ECHCS). The government is seeking to replace existing fragile units with more durable and cost-effective borescopes to support Sterile Processing Service operations.

• Six (6) Borescope Inspection Kits.

• Each kit must include: MOLE IPV Control or equal, MOLE RM Camera (quantity 2) or equal (one 98" camera 1.3 mm diameter, one 39" camera 1.3 mm diameter), and MOLE USB A-C cable interface or equal.

• Salient Characteristics include:

  • Non-fiber, video-based imaging.
  • Two borescopes: 39-inch (1.3 mm diameter) and 98-inch (1.3 mm diameter).
  • Optional 1.0 mm diameter, 59-inch borescope.
  • Software independence (plug-and-play via USB with standard Windows).
  • Integration with instrument tracking quality feedback workflows (e.g., Censis).
  • Waterproofing (IPx7 or higher).
  • Modular, field-replaceable components.
  • Adjustable/automatic white balance for true-color images.
  • Compatibility with VHP sterilization (ASP STERRAD®, STERIS VPro®).

• Delivery within 30 days from award to Rocky Mountain Regional VA Medical Center, Aurora, CO.

• Equipment must be new, not used or refurbished, with specific date of manufacture limitations.

• Vendor must provide service manuals, maintenance requirements, and part lists.

• Anticipated contract type: Firm-fixed price.

• Award will be made on an All or Nothing Basis.

• Questions Deadline: June 23rd, 2026, 12:00 MT (submit in writing).

• Quote Submission Deadline: June 26th, 2026, 14:00 MT (email only to eric.adusei@va.gov; 10MB email limit).

• Submission Volumes:

  • Volume I: Technical capability demonstrating meeting of SON and solicitation requirements.
  • Volume II: Price (completed pricing schedule).
  • Volume III: VAAR 852.219-75 (Limitations on Subcontracting) and FAR 52.225-2 (Buy American Certificate).

• Evaluation Factors:

  • Factor I: Technical capability or quality.
  • Factor II: Price.

• Offers not meeting technical requirements will not be considered, regardless of price.

• This solicitation is 100% set-aside for Service-Disabled Veteran-Owned Small Businesses (SDVOSB).

• SDVOSB status must be verified in SBA Small Business Certification (SBS) at the time of quote submission and award.

• NAICS Code: 339112 (Surgical and Medical Instrument Manufacturing) with a size standard of 800 Employees.

• Reference attached Statement of Need (SON) for full salient characteristics.

• Reference attached 52.225-2 BUY AMERICAN CERTIFICATE and 852.219-76 VA NOTICE OF LIMITATIONS ON SUBCONTRACTING.

• Offerors are warned against contacting VA personnel other than the Contracting Officer.

This document is the "Buy American Certificate" provision (FAR 52.225-2). It requires offerors to certify the origin of end products and their components to comply with the Buy American Act.

• Offeror Certification: Requires certification that each end product is a domestic end product, with exceptions listed.
• Foreign End Products: Section (b) requires listing foreign end products, their country of origin, and whether they exceed 55% domestic content (unless COTS).
• Domestic End Products with Critical Components: Section (c) requires listing domestic end products that contain a critical component.
• Definitions: Refers to FAR 25.105 and the "Buy American—Supplies" clause for definitions of key terms.

Bidders must complete and submit this certification as part of their offer. Failure to properly certify the origin and domestic content of end products may impact the evaluation of their offer under the Buy American Act policies and procedures outlined in FAR Part 25.
