Statement of Work (SOW)

To procure a brand name only compact eye exam lane for the DC VA Medical Center Eye Clinic to equip a new examination room (1E302) for patient vision checks, autorefraction, and slit lamp examinations.

Procurement includes all necessary parts, materials, labor, software/licenses, resources, and training to implement and utilize the equipment. All items must be covered by manufacturer's warranty and sourced through a manufacturer-approved distribution channel. Distributors and resellers must document their ability to provide items through such channels.

• Specific features for KR-800s Auto Kerato-Refractometer (subjective visual acuity, various measurement ranges and modes).

• Specific features for SL-D701 Slit Lamp (multiple magnification levels, LED illumination, various filters, variable slit width/length, X-Y-Z base movement).

• Specific features for SOLOS Auto Lensmeter (automated prescription measurement, specific sphere, cylinder, axis, add power, and prism ranges).

• Delivery Location: Washington DC VA Medical Center, 50 Irving St NW, Washington, DC 20422, ATTN: VHAWAS Equipment Committee.

• Delivery Timeline: Within 90 days from contract award.

• Delivery Hours: Monday-Friday, 8:00 a.m. to 4:00 p.m. (excluding Federal holidays).

• Contractor must coordinate delivery with the on-site Point of Contact (POC).

• Installation by certified and knowledgeable personnel.

• Contractor responsible for PPE.

• All removable media must be scanned for viruses.

• Damaged government property must be reported and replaced at contractor's expense.

• On-site training for clinical staff/users, scheduled in advance.

• Joint inspection and acceptance upon completion of installation and addressing any deficiencies.

• Service report.

• Warranty information.

• Electronic copies of Operator's Manual and complete technical service manuals.

This is a brand name only request for Topcon equipment. The facility currently uses Topcon KR-800S Auto Kerato-Refractometers and SOLOS Auto Lensmeters and seeks to purchase the same models to ensure uniformity in patient care, reduce training needs for staff (approx. 25 members), and simplify cleaning/disinfection procedures. Topcon is ERA approved for network connectivity.

Solicitation / RFP / RFQ

This is a combined synopsis/solicitation (RFQ) for commercial items to procure a brand name only compact eye exam lane for the DC VA Medical Center Eye Clinic. The government intends to award a single Firm-Fixed Price (FFP) contract.

• The core requirement is a brand name only compact eye exam lane.

• Offerors must demonstrate that their proposed equipment meets the salient physical, functional, or performance characteristics specified in the solicitation (see Attachment A).

• Non-compliance with these characteristics will result in a non-responsive quote.
