• Attachment A: Updated Bill of Materials.

• Attachment B: Updated Line Item Spreadsheet, specifically regarding packaging.

• The deadline for quotes has been extended to April 13, 2026, at 8:00 AM EST.

Questions & Answers / Clarifications

This document provides clarifications and answers to vendor questions regarding the Custom Surgical Packs solicitation (36C24626Q0364).

• Past Performance Disclosure: Vendors have discretion on which contract numbers or proprietary information to disclose for past performance evaluation. The government is not requesting proprietary data but uses this information to assess experience on similar contracts.
• Supplier vs. Subcontractor: Vendors must clarify if they supplied the procured items themselves or through a subcontractor.
• Awardee vs. Subcontractor: Vendors must state whether they were the prime awardee or a subcontractor on the referenced contract.
• TAA Compliance: Clauses 52.225-5 and 52.225-6 apply. Provision 52.225-6 must be completed for U.S.-made or designated country end products.
• Price/Cost Schedule Consolidation: Quantities in Section B.2 will not be combined by facility; they will remain listed by location (Salisbury, Charlotte, Kernersville).
• Attachment B, Column BZ: This column requires the vendor's part number. If it's the same as the manufacturer's part number, enter that.
• Packs per Case: Information on the number of packs per case is available in the revised Attachment B (provided in amendment 0004).
• Components per Pack: The quantity of components per pack is detailed in Attachment A – Bill of Materials.

These clarifications address vendor concerns regarding past performance, TAA compliance, and data entry for attachments. Vendors should ensure their proposals reflect these clarifications, particularly regarding the Price/Cost Schedule structure and attachment details.

• Item Descriptions: Various custom surgical packs (e.g., CYSTO, ENT, LAP CHOLE, MAJOR LAPAROTOMY, MINOR PROCEDURE, KNEE ARTHROSCOPY, SHOULDER, PODIATRY, TOTAL KNEE-LF, TOTAL HIP-LF, 21 ORTHO) and custom kits (PRE-OP 1, PRE OP 2 W/BAIR PAW, CLINIC PROCEDURE, EYE KIT).
• Quantities: Specific quantities are listed per item, often broken down by delivery location (Salisbury VAMC, Charlotte HCC, Kernersville HCC).
• Contract Periods: Includes Base period (07/01/2026 - 06/30/2027), Option 1 (07/01/2027 - 06/30/2028), Option 2 (07/01/2028 - 06/30/2029), and Option 3 (07/01/2029 - 06/30/2030), and Option 4 (07/01/2030 - 06/30/2031).
