This solicitation is for custom surgical procedure packs for the Salisbury VA Medical Center and all VA clinics within the Salisbury system. The government requires these packs to be assembled, sterilized, and delivered in compliance with federal, state, and local regulations.

• Custom surgical procedure packs for various surgical types (e.g., Cataract, Knee, Hip, Arthroscopy, Chest, Ear Nose and Throat, Endourology, Hand, Robot-assisted, Podiatry, Vascular, Endovascular, Minor/Clinic procedures).

• Packs must be assembled in secure, air-controlled rooms.

• Sterilization must meet ISO 9001:2015 standards and follow FDA, HSPA, and AAMI guidelines.

• Components must be Latex Free unless otherwise specified and clearly labeled.

• Packs must be packaged in rigid plastic base trays to reduce ETO residue and eliminate particulate contamination.

• All packs must be clearly labeled with manufacturing date, sterilization date, expiration date, and component list.

• A minimum shelf life of ninety (90) calendar days remaining upon delivery is required.

• Compliance with Trade Agreements Act (TAA) is required for all components unless otherwise authorized.

• The contract period includes a base year plus four option years.

• Ordering period for delivery orders is from 09/01/2026 through 08/31/2031.

• Quotes must be submitted electronically via email to Aleshia.Hernandez@va.gov by 06-30-2026 at 10:00 am EDT.

• Quotes must include a signed SF 1449, completed Attachment B (Price), a technical description, and past performance information.

• Evaluation will be based on Technical, Past Performance, and Price.

• The government reserves the right to select a quotation that provides the best value.

• This acquisition is UNRESTRICTED.
