Solicitation / RFP / RFQ

This is a Sources Sought Synopsis and Request for Information (RFI) for custom surgical packs for the Department of Veterans Affairs (VA) Veterans Integrated Service Network (VISN) 22. The VA is seeking potential sources capable of supplying custom packs for scheduled procedures in the Operating Room, Interventional Radiology, and Cardiac Catheterization Lab at the VA Greater Los Angeles Healthcare System and VA Long Beach Healthcare System. This notice is for information and planning purposes only and does not constitute a solicitation. A solicitation is not currently available, but if issued, will be announced later.

• Supply custom surgical packs tailored to specific needs for scheduled procedures.

• The contract will be a firm-fixed price, IDIQ Base Plus Four Year Contract.

• Custom packs require assembly/manufacture, sterilization, quality assurance, and packaging.

• Initial orders for custom packs must be produced, sterilized, and delivered within 24 weeks.

• Contractor must provide all labor, materials, transportation, equipment, and supervision.

• Specific pack types and estimated quantities are listed for both VALBHS and VAGLAHS (refer to attachments for detailed Bills of Materials).

• Contractor must maintain a 3-month supply on hand and achieve a 95% acceptance rate.

• Packs must have a minimum shelf life of twelve (12) months at the time of delivery.

• Contract Type: Firm-fixed price, IDIQ Base Plus Four Year Contract.

• Period of Performance: One (1) one-year base period plus four (4) one-year option periods.

• This is an RFI/Sources Sought, not a solicitation. Responses are not offers.

• Interested parties should submit vendor information including: Vendor Name, Address, Telephone Number, Business Size, Socioeconomic Category, UEI Number, FSS/GSA Contract details (if applicable), and Country of Origin.
