POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONSE:`
1. Company name, address, phone number, primary contact(s), e-mail address.
2. State if your firm distributes the Brand Name items, or Equal items.
3. SAM UEI number.
4. NAICS (North American Industry Classification System) code 339112 that your firm uses for injection pumps and modules.
5. State if your firm is a large business, small business, service-disabled-veteran-owned
small business, veteran-owned small business, HUB zone 8(a), women-owned small business or a small disadvantaged business.
6. State if your firm has the required items on Schedule (FSS, GSA, SEWP, other ).
7. Provide a letter from the manufacturer verifying your firm is an authorized distributor.
8. Provide the country of origin for each of the required items.
REQUIRED ITEM and Salient Characteristics:
Description & Salient Characteristics: The Sioux Falls VA needs to replace its aged and outdated surgical simulator systems with brand name or equal to the Surgical Science/Simbionix ENDO SUITE Simulator and LAP Mentor III. ENDO Suite Simulator covers multiple endoscopic specialties including Gastrointestinal (GI) endoscopy, Bronchoscopy, and Endourology. The system offers realistic training with feedback using flexible scopes to view the digestive tract, airways, and urinary tracts. Lap Mentor III focuses on laparoscopy focusing on hands-on skills, such as camera navigation, instrument manipulation, and suturing.
ENDO Suite Simulator
Must have the ability to use the listed Simbionix Mentor Modules already owned by the facility to save on costs:
Gastric Bleeding (Basic)
Endoscopic Cholangiopancreatography (ERCP),
Flexible Sigmoidoscopy
Endoscopic ultrasound (EUS)
Essential Bronchoscopy
Diagnostic Bronchoscopy
Essential Endobronchial Ultrasound (EBUS) Modules
Must have a GI Endoscopy Fundamental Skills, and Endoscopic Mucosal Resection (EMR), and Endoscopic Submucosal Dissection (ESD) modules.
Must have software that allows for curriculum management, online learning, and results monitoring.
Must have service plan that offers 24/7 technical and phone repair support, and software updates.
This must include installation and training.
Must give trade in allowance on current GI-BRONCH Mentor system.
LAP Mentor III
Must have the ability to use the listed Simbionix Mentor Modules already owned by the facility to save on costs:
Basic Skills: Essential Tasks
Basic Skills: Suturing, Basic, and Advanced including suture handles
General Surgery: LAP Chole Procedural Tasks and Full Procedures
General Surgery: Inguinal Hernia, Appendectomy Tasks, Techniques, and Full Procedures
Bariatric Surgery: Gastric Bypass
Urology/Renal Surgery: Nephrectomy
GYN: Essentials
Must have VR headset and with case support for Inguinal Hernia, Appendectomy, and ability to add any future supported cases.
Must have Cholangiography, Incisional Hernia, Nissen Fundoplication, Distal Gastrectomy, and Sigmoidectomy modules.
Must have software that allows for curriculum management, online learning, and results monitoring.
Must have service plan that offers 24/7 technical and phone repair support, and software updates.
This must include installation and training.
Must give trade in allowance on current LAP Mentor system.
