Quick Summary

The Department of Veterans Affairs (VA) has awarded a firm fixed-price Purchase Order to O R Elder Inc. for three (3) Unity VCS VLTP/Surgery machines and associated handpieces for the Ophthalmology department at the LTC Charles S. Kettels VA Medical Center in Ann Arbor, MI. This award, valued at $675,624.00, was made on April 6, 2026, under a sole source justification due to the unique capabilities of the Unity VCS system and the critical need to replace end-of-life equipment.

Scope of Work

This procurement includes:

  • Three (3) Unity VCS VLTP machines, complete with VCS consoles, remote, laser, large tray, various footswitches (VCS 6-button, CS 4-button, Laser), BSS Rack, 2 UNITY 4D Phaco handpieces, and an Ozil Frag handpiece.
  • Twenty-seven (27) ALCON 4D Phaco Handpieces. These vitrectomy machines are crucial for addressing severe and vision-threatening ophthalmological issues, including retinal detachments, diabetic retinopathy complications, macular holes, and eye trauma.

Contract & Timeline

  • Type: Firm Fixed-Price Purchase Order

  • Award Amount: $675,624.00
  • Award Date: April 6, 2026
  • Awardee: O R Elder Inc. (a Service-Disabled Veteran-Owned Small Business - SDVOSB)

  • Delivery: Anticipated within 1 month from receipt of order.

Justification for Sole Source

The Ann Arbor VA Health Care System's existing Alcon Centurion machines are at end-of-life, with one currently non-operational. The Unity VCS system offers unique, advanced technology not available from competitors, including a dual peristaltic pump with Venturi option, nitrogen-independent function, 4D Phaco for faster cataract removal with less energy, Unity Thermal Sentry for corneal burn prevention, Unity Hypervit (fastest vitrectomy probe), and Unity TetraSpot (multi-spot laser). It is also a combined anterior and posterior segment machine, reducing the operating room footprint and potential repair delays. The absence of a working backup machine poses significant risks to patient safety and access, potentially leading to irreversible vision loss for veterans. The OEM, Alcon, confirmed that O R Elder Inc. is the exclusive source for this capital equipment.

Market Research & Competition

Market research for NAICS 334510 (1250 employee size standard) identified 635 VOSBs, but zero results when "Vitreoretinal Cataract System" was added. O R Elder Inc. was separately verified as an SDVOSB. Prior purchases of this system since 2016 were exclusively from O R Elder. Searches on GSA Advantage and NAC yielded no results. A previous sources sought notice (36C25626Q0440) for a similar requirement also received only one response from O R Elder. The VA and SBA rules of two are not attainable for this procurement. The agency plans to survey the marketplace for future procurements to identify potential barriers to competition.

Additional Notes

The Buy American Act applies, and the products are made in the United States.
