Statement of Work (SOW) / Performance Work Statement (PWS)

To procure examination tables, chairs, and white glove installation services for the new VA Puget Sound Healthcare System Auburn Primary Care Clinic. This will enable medical personnel to function properly while supporting patients during examinations, treatments, and procedures.

• Provide examination tables, chairs, and associated equipment.

• Furnish all OEM authorizations, licenses, uniforms, equipment, tools, materials, supplies, storage, insurances, shipping/delivery costs, and transportation.

• Perform "white glove" installation, including planning, operations, delivery, manpower, and installation.

• Provide electronic, digital copies of user and repair manuals.

• Product Delivery: Auburn CBOC, 1002 15th Street SW, Auburn, WA 98001.

• Delivery Schedule: No more than 40 days after receipt of order (ARO) and/or between 03/24/2026 to 05/03/2026.

• Installation: To be coordinated with the VA POC post-award.

• Specific quantities of items including: Chairs, Barrier Free Exam; Exam LED Extended Spline Mount System; Chairs with Digi Scales and Pelvic Tilt; Patient Support Rails; Premium Upholstery items; Procedures Tables; Mobile Carts for Vital Signs Devices; Wall Mounts for Vital Signs Devices; Digital Vital Signs Devices; Wall Systems; Tip Dispensers; Instrument Panels; Procedure Lights; Basic Stools; Bariatric Side Chairs; Workstations.

• "White Glove" delivery and installation service.

• Products must meet or exceed specified physical, functional, and performance characteristics.

• Safety features for exam tables (lockable casters, safety straps, programmable foot controls, adjustable height, safety switches, obstruction detection).

• Maintenance requirements for tables, chairs, and lights (cleaning, upholstery care, electrical wiring inspection, repairs, bulb replacement).
