Statement of Work (SOW)

The objective is to procure and install a GE Healthcare MAC 5 A4 system to enhance the efficiency and effectiveness of the Home-Based Primary Care (HBPC) program at the VA of Central Iowa Health Care System.

The contractor shall provide all labor, equipment, materials, and supervision for the delivery and installation of the GE Healthcare MAC 5 A4 system. Key tasks include:

• Delivery and installation to the specified VA location in Des Moines, IA.

• System configuration, including software and hardware setup.

• Comprehensive testing to ensure correct and efficient operation.

• On-site training for designated HBPC staff on system use, maintenance, and troubleshooting.

• Provision of training materials and user manuals.

• Submission of detailed installation documentation, including diagrams, configurations, and test results.

• Performance Period: Expected completion within 30 days from the contract award date.

• Place of Performance: VA of Central Iowa Health Care System, 3600 30th Street, Des Moines IA, 50310.

• Contractor Qualifications: Must have relevant experience with GE Healthcare MAC 5 A4 systems or similar equipment. Technicians must be manufacturer-certified.

• Quality Assurance: A quality assurance plan is required to meet industry standards and manufacturer specifications.

• Safety and Compliance: Adherence to all applicable local, state, federal regulations, OSHA standards, and VA policies is mandatory.

• Fully installed and operational GE Healthcare MAC 5 A4 system.

• Training session(s) for HBPC staff.

• Installation and configuration documentation, test results, and user manuals.

Successful installation, configuration, testing, staff training, and delivery of all required documentation.

Request for Information / Sources Sought

This notice is a Request for Information (RFI) / Sources Sought for Portable EKG's, Brand Name or Equal, to support the Central Iowa VA Healthcare System (HCS). The Department of Veterans Affairs, VISN 23, is seeking qualified business sources that can provide some or all of the listed items. This is NOT a solicitation.

The RFI seeks information on the ability to provide Portable EKG's. Specific requirements for potential sources include:

• Confirmation of distributing Brand Name items or Equal items.
• SAM Unique Entity Identifier (UEI) number.
