Statement of Work (SOW) / Performance Work Statement (PWS)

This draft Statement of Work (SOW) outlines the requirements for replacing a Falcon Peripheral Vascular Diagnosis System, or an "equal" system, due to the current equipment being unsupported by the manufacturer. The system is frequently used for diagnosing peripheral vascular abnormalities.

• The VA Loma Linda is requesting contractors to provide a brand name or equal equipment that meets or exceeds the salient characteristics listed.

• The equipment must be capable of diagnosing peripheral vascular abnormalities.

• System Capabilities:

  • At least 10 Independent Pressure Channels for simultaneous blood pressure measurements in various limb segments.
  • At least 5 independent Photoplethysmography (PPG) sensors for diagnosing conditions like Raynaud's syndrome.
  • A 10mhz (+/- 1mhz) Doppler probe for measuring small peripheral blood vessels.
  • Ability to quickly measure Ankle Brachial Index (ABI) and Thoracic (TBI).
  • A user-friendly interface supporting unlimited configurable user-defined protocols and reports.

• Installation and Training: Contractor must provide all necessary tools, parts, materials, equipment, travel, and labor for installation. An on-station in-service and training on proper use and setup is required.

• Warranty: Standard manufacturer industry warranty term is required. Extended warranties requiring advance payment are not accepted.

• Delivery: Items are to be delivered and/or installed after receipt of purchase order. Delivery location will be confirmed post-award and must be coordinated with the VA POC prior to any delivery attempts.

• Vendor Responsibilities: Includes offloading, moving, uncrating, unboxing, trash removal, and setting up the equipment according to manufacturer specifications. Equipment installation paperwork and warranty information must be provided.

• Shipping: All shipping costs for parts are included in the contract cost.

• Place of Delivery: VA Loma Linda Healthcare System, 11201 Benton Street, Loma Linda, CA 92357-1000.

• Delivery/Installation: Anticipated after receipt of purchase order.

• Security: Vendor must ensure adequate LAN/Internet, data, information, and system security in accordance with VA standard operating procedures. Personnel requiring access to VA computer systems must undergo a background investigation and receive favorable adjudication.

• Holidays: The contract observes specific federal holidays for duty hours.

• Non-contract Charges: No services resulting in additional charges are permitted without prior Contracting Officer approval.

• Payment: Payment will be made upon receipt of a properly prepared invoice.

• Waste Removal: Contractor is responsible for removing and disposing of all waste related to services provided on-site.

• This SOW is tentative and subject to change based on responses received for the RFI.
