This document is the "Buy American Certificate" provision (FAR 52.225-2, Oct 2022). It requires offerors to certify the domestic content of end products and critical components proposed for the opportunity. This form is used to document compliance with the Buy American Act.

Bidders must complete and submit this certificate as part of their proposal. It is crucial for determining the eligibility of proposed products under the Buy American Act and will be used by the Government for offer evaluation in accordance with FAR Part 25.

Statement of Work (SOW) / Performance Work Statement (PWS)

To procure and deliver Natus Sensory Boothless Audiometry equipment and associated components to the VA Eastern Colorado Health Care System in Aurora, CO.

• Deliver all items listed in section 2.5, meeting the defined salient characteristics.
• Provide all necessary personnel, supplies, and equipment for delivery.
• Furnish two (2) binders and one (1) digital copy of operation and maintenance manuals for all items upon installation completion.

This is a BRAND NAME OR EQUIVALENT requirement, with the Natus brand as the basis of design. Specific items and their salient characteristics are detailed, including:

• MADSEN Astera2 (or equivalent): Features include dual channels, advanced audiometry (TEN, SAL tests), speech tests, specific frequency ranges (up to 20000 Hz), various masking types, and precise sound level accuracy.

• ACP ASTERA 2 (or equivalent): Specific model number provided.

• MISCO Speaker System (or equivalent): Full-range speaker with specified impedance, power handling, and frequency response.

• Quicksin Material US (or equivalent): Audiological test tool CD for speech comprehension in noise.

• PMM CS W HI-PRO US (or equivalent): Hardware interface for hearing aid programming and verification.

• AuDX Pro FLEX (or equivalent): Multifunction device for tympanometry, OAE, and audiometry, with a 5.0" touchscreen display.

• Bio-logic Diagnostic DPOAE License for Flex (or equivalent): Software license for DPOAE testing.

• RE-VO2 Video Otoscope + Cradle (or equivalent): Handheld digital otoscope with imaging capabilities and a cradle.

• 4mm single-use specula (or equivalent): Disposable specula for compatible otoscopes.

• Delivery Timeframe: Within 30 days of award, unless otherwise specified.

• Delivery Location: VA Eastern Colorado Health Care System, 1700 N. Wheeling Street, Aurora, CO 80045.

• Warranty: All manufacturer warranties for parts and labor, including travel and shipping for repairs, must be provided at time of delivery.

• Delivery Markings: Items must be in original sealed containers, marked with manufacturer's name, PO, and contract number.

• Manuals: Two (2) binders and one (1) digital copy of operation and maintenance manuals are required upon installation.

This notice is for market research purposes to determine industry interest and capability for a Sensory Boothless Audiometry System. The Department of Veterans Affairs (VA), Network Contracting Office 19 (NCO 19), is seeking potential sources for delivery to the Eastern Colorado VA Healthcare System in Aurora, CO.
