This document is the Buy American Certificate provision (Oct 2022) that offerors must complete and submit as part of their proposal. It requires certification regarding the origin of end products and their components.

• Certification: Offerors must certify that each end product is a domestic end product, except for those listed in paragraph (b).
• Foreign End Products: Offerors must list any foreign end products, their country of origin, and indicate if they exceed 55% domestic content (unless they are COTS items).

This form is crucial for compliance with the Buy American Act. Bidders must accurately identify the origin and domestic content of all end products included in their offer. Failure to complete this certification correctly may impact the evaluation of the offer. The government will evaluate offers in accordance with FAR Part 25.

Statement of Work (SOW) / Performance Work Statement (PWS)

The Department of Veterans Affairs (VA), Eastern Colorado Health Care System (ECHCS), requires the procurement of one specialized urodynamic examination and procedure chair for the Urology Service at the PFC Floyd K. Lindstrom Clinic.

• Provide one (1) specialized urodynamic examination and procedure chair.

• The chair must support diagnostic testing of bladder and urethral function, including uroflowmetry, cystometrogram, pressure-flow studies, electromyography, and urethral closure testing.

• The contractor shall provide all necessary equipment, accessories, delivery, installation, documentation, warranty, and support for a fully operational chair.

• Any proposed equal product must meet or exceed all salient characteristics and be fully compatible with clinical workflows without additional Government cost.

• Specifically designed for urodynamic/urologic procedures.

• Electrically powered adjustments for height, backrest, seat tilt/pelvic elevation, and leg supports.

• Includes hand and foot controls for hands-free positioning.

• Supports upright, lithotomy, and near-horizontal/supine positioning.

• Maximum entry height not to exceed 22 inches.

• Minimum patient weight capacity of 500 pounds.

• Factory-designed perineal cutout suitable for catheterization and uroflowmetry.

• Adjustable leg supports, stirrups, or footrests.

• Compatible funnel holder for urine flow measurement.

• Stable platform during powered movement and invasive testing.

• Medical-grade cleanable, fluid-resistant surfaces.

• Operates on standard U.S. hospital electrical power (120 VAC).

• Standalone device; no network connectivity required.

• New, current-generation equipment; refurbished items not acceptable.

• Manufacturer warranty minimum one year.

• Onetime equipment purchase. Warranty begins upon Government acceptance.

• Place of Performance: Rocky Mountain Regional VA Medical Center, 1700 North Wheeling Street, Aurora, CO 80045.

• Delivery shall be FOB Destination to the specified Aurora, CO address.

• Delivery Hours: Monday – Friday; 0700hrs – 1500hrs, excluding Federal Holidays.

• Installation shall be performed by OEM-certified or OEM-authorized personnel, including full system assembly, integration, and configuration.

• Operational and functional verification required post-installation.

• Acceptance contingent upon successful installation, functionality verification, component delivery, and receipt of documentation.

• Items must be delivered in original sealed containers, marked with Purchase Order Number, Contract Number, and "Mark For Attention: Rhonda Burton".

• Provide current OEM operation and maintenance manuals.

• Basic operator orientation at installation, covering system operation, safety, and procedures.

• Minimum one-year manufacturer warranty covering parts and labor.

• Extended warranty options shall be available.

Solicitation / RFP / RFQ

This is a combined synopsis/solicitation (RFQ #: 36C25926Q0406) for commercial items, specifically an Urodynamic Exam Chair, for the Department of Veterans Affairs, Rocky Mountain Regional VA Medical Center in Aurora, CO. The government intends to award a firm-fixed-price contract.

• Offerors must provide new OEM medical supplies; remanufactured or gray market items are not acceptable.

• Offerors must be an OEM, authorized dealer, distributor, or reseller, verified by OEM documentation.

• Contract Type: Firm-Fixed-Price

• Delivery: F.O.B. Destination to Rocky Mountain Regional VA Medical Center, Aurora, CO 80045.

• Response Date/Time: May 15, 2026, 2:00 PM Mountain Time, Denver, USA.

• Questions Deadline: May 12, 2026, 10:00 AM Mountain Time.

• Submission Method: Email quotes only to eric.adusei@va.gov (4MB email limit). Faxed or hand-carried quotes will not be accepted.

• Evaluation Factors:

  1. Technical capability or quality.
  2. Price.
  3. Completion of "Attachments – RFO 52.225-2 Buy American Certificate".

• Award will be made on an "All or Nothing" basis.

• The government may award based solely on initial quotes received.

• This procurement is a 100% set-aside for Small Business (SB) Vendors.

• NAICS Code: 339112 (Surgical and Medical Instrument Manufacturing) with a size standard of 1000 Employees.

• Offerors must be SBA certified and visible in the SBA Database if the set-aside is for SDVOSBs/VOSBs (though this is a general SB set-aside).

• The solicitation is prepared in accordance with FAR Part 12.

• All questions must be submitted in writing by the deadline; answers will be provided via amendment.

• Offerors must submit specifications, cut sheets, or brochures confirming "equal" products meet or exceed requirements, along with proof of Authorized Distributor status.

• A Buy American Certificate (Attachment – RFO 52.225-2) must be completed and submitted.

• Clause 52.222-90 (Addressing DEI Discrimination by Federal Contractors) applies.
