This is a Request for Quotation (RFQ) for Dental Unit Waterline Testing Services for the Department of Veterans Affairs, Oklahoma City VA Health Care System. • Provide mail-order testing kits to VA Dental Assistants.

• Perform laboratory analysis of dental unit waterline samples for total colony forming units (CFU).

• Provide lab test reports in electronic format (online or email) within one week of receiving samples.

• Offer retesting if needed.

• Contractor must be located within a reasonable distance to prevent extended shipping times.

• Contractor must comply with "Infection Control Standards for VA Dental Clinics" (2013) and any future updates.

• Contractor must be certified (EPA or state) to perform environmental testing.

• Contractor must demonstrate service performance in accordance with all applicable laws and regulations.

• Contractor must be able to commence services within 5 business days of task order issuance.

• Contract Type: Indefinite Quantity, Indefinite Delivery (IDIQ), Firm-Fixed-Price.

• Period of Performance: Contract award to September 2028.

• Estimated Quantities: Approximately 855 dental unit waterline samples per 12 months.

• Maximum Aggregate Ceiling Value: To be inserted upon award.

• This acquisition is a total small business set-aside.

• The solicitation includes numerous FAR and VAAR clauses, including those related to security, subcontracting limitations, and electronic payment submissions.

• A Wage Determination (WD) is included in Section D, outlining minimum wage rates and fringe benefits for various occupations.

• Limitations on Subcontracting: Contractor will not pay more than 50% of the contract amount to firms that are not certified SDVOSBs.
