This solicitation is for a multiple award Blanket Purchase Agreement (BPA) to provide optical eyeglass lenses. The requirement is for the Department of Veterans Affairs (VA), Richard L. Roudebush VAMC, located in Indianapolis, Indiana. The BPAs will support the VISN 10/12 Optical Laboratory and associated facilities across Indiana, Illinois, Michigan, Ohio, and Wisconsin.

• Provision of blank ophthalmic eyeglass lenses (blank pucks) for fabrication.

• Lenses must be fabricated in accordance with ANSI Z80.5-2004 standards.

• All blank eyeglass lenses require a one-year manufacturer's warranty.

• Contractors must resolve and replace manufacturing defective lenses at no cost to the Government.

• Delivery of ordered items within four business days of receipt of a delivery order.

• Notification to the Optical Fabrication Laboratory within 24 hours if unable to meet delivery dates.

• Quarterly reports of lenses procured, including model numbers, quantity, and dollar value.

• Potential transition to digital lens designs during the contract period.

• Period of Performance: Five (5) year period from date of award.

• Estimated Quantities: Base year estimated at 178,383 pairs, with an approximate 5% annual increase. Quantities are estimates and not guaranteed.
