This document is a Justification and Approval (J&A) for Other Than Full and Open Competition for the establishment of an Indefinite Delivery, Indefinite Quantity (IDIQ) contract. The contract will procure Cepheid test kits (reagents) for use with existing Cepheid GeneXpert DX instruments at the VA Southern Nevada Healthcare Center (VASNHCS) laboratory. The action is being procured as a sole source because the test kits are only available through the Original Equipment Manufacturer (OEM), Cepheid Inc., due to proprietary rights.

The contract will procure various Cepheid test kits, including but not limited to:

• GXCARBAR-10 (detection of carbapenem resistance)

• GXCDIFF/EPI-10 (detection of Clostridium difficile)

• GXMRSA-NXG-10 (detection of MRSA)

• GXCT/NG-10 (detection of Chlamydia trachomatis)

• XPRSTREPA-10 (detection of Strep A)

• XPRS4PLEX-10 (detection of SARS-CoV-2, influenza A, influenza B, and/or RSV)

• XPRSMVP-10 (detection of DNA from BV organisms, Candida species, and Trichomonas vaginalis)

• 900-0370 (Nasal Swabs)

• SWAB/G-50 (various swabs)

• URINE/A-50 (urine collection kit)

• 3C057N (transport medium)

• SDPS-120 (Specimen Collection Swab)

• 480CFA (liquid based collection and transport swab)

• CARBAR-BU1 (detection of carbapenemase producing Enterobacterales)

• Contract Type: Firm Fixed Price IDIQ

• Period of Performance: Anticipated from 06/01/2026 to 09/30/2030, with an option to extend for an additional 6 months.

• Anticipated IDIQ Ceiling: $6,300,000.00

This document is a justification for sole source procurement, not a solicitation. Therefore, proposal submission instructions and evaluation criteria are not detailed herein.

This procurement is restricted to a sole source due to proprietary rights and the unique nature of the test kits required for the existing Cepheid GeneXpert DX instruments. No set-aside is planned.

• The VA owns the Cepheid GeneXpert Infinity 48 module analyzer, and staff are trained on its use.
• Testing occurs 24/7 on a STAT basis, requiring results within 60 minutes.
• Market research confirmed no other distributors or manufacturers of these specific proprietary kits.
• Previous sole source contract (36C26124D0063) was awarded for these kits.

This is a pre-solicitation notice of intent to award a sole source, firm-fixed-price Indefinite Delivery Indefinite Quantity (IDIQ) contract to Cepheid. The contract is for molecular/testing reagents to be used at the VA Southern Nevada Healthcare System.

• The contract will be for test kits (reagents).

• Contract type: Firm-fixed-price IDIQ.

• Period of Performance: FY26.

• The solicitation will be posted on SAM.gov on or about March 31, 2026.

• Interested parties must submit a capability statement, proposal, or quotation.

• The government will consider all responsible sources.

• The agency will determine whether to proceed with the sole source acquisition after consideration of submissions.

• Responses are requested in writing via email to Mathew.Czeshinski@va.gov by 12:00 EST on March 31, 2026.

• The notice indicates an intent for a sole source acquisition, but states that all responsible sources may submit a capability statement, proposal, or quotation, which shall be considered.
