THIS IS A SOURCES SOUGHT NOTICE ONLY. This is not a solicitation for bids, proposals, proposal abstracts, or quotations. The purpose of this Sources Sought Notice is to obtain information regarding the availability and capability of all qualified sources to perform a potential requirement. The responses received from interested contractors will assist the Government in determining the appropriate acquisition method. The Department of Veterans Affairs (VA), NCO 08- is conducting a market research survey to identify potential qualified sources capable of providing Chemistry Cost Per Reportable Result (CPRR) and Automation System at a firm fixed unit price for the following locations:
Bay Pines VA Healthcare System, CW Bill Young C.W. Bill Young VA Medical Center 10,000 Bay Pines Blvd, Bay Pines, Florida 33744
Bay Pines VA Healthcare System, Lee County VA Healthcare Center (LCHCC) 2489 Diplomat Parkway East, Cape Coral, Florida 33909

Interested parties should provide the following information:
1. Company name, address, point of contact, and point of contact email address and phone number.
2. UEI number and CAGE code.
3. Business size status under the applicable NAICS code.
4. Existing government Contract Number/ Contract for scope and expiration date.
5. Manufacturing country of origin.
6. Authorized distributorship letter from the manufacturer (If a distributor)
7. Will the prime contractor provide products it manufactures?
8. Will the prime contractor acquire the products from a small or large business manufacturer?
9. If it is a nonmanufacturer, is it providing the end items of a small business manufacturer?
10. What percentage of the total contract value will be subcontracted?
11. How much transition time is needed?

This is a Sources Sought Notice and submissions will be used for informational and planning purposes only.
