Statement of Work (SOW) / Performance Work Statement (PWS)

To establish a contract for the supply of medical cylinder gases to the VA Greater Los Angeles Medical Center at the West Los Angeles (WLA) and Sepulveda (SEP) Campuses.

• Contractor must provide verification of FDA and state licensing/permits annually, or provide manufacturer's documentation if not the manufacturer.

• Delivery to specific lab locations within WLA and SEP campuses, with potential for additional locations in the Greater Los Angeles area.

• Contractor is responsible for swapping out old tanks upon delivery.

• Emergency orders must be filled within 24 hours of notification.

• Invoices are to be submitted monthly.

• All products, equipment, and services must comply with applicable Federal, state, and local regulations.

• Licenses, certifications, and insurance must be provided within 10 days of contract award and updated annually.

• Material Safety Data Sheets (MSDS) to be provided upon request.

• VA Greater Los Angeles Medical Center (WLA and SEP Campuses).

• A 30-day period between contract award and effective/start date is anticipated.

• Cylinders must comply with Department of Transportation (DOT) specifications and CGA standards for valves and marking.

Solicitation / RFP / RFQ
