Statement of Work (SOW) / Performance Work Statement (PWS)

To procure specialized physical therapy equipment for wheelchair-bound patients at the VA San Diego Spinal Cord Injury (SCI) and Community Living Center (CLC).

Contractors are to provide specific brand-name physical therapy equipment that meets or exceeds the listed salient characteristics. The equipment is essential for rehabilitative therapy services for patients in wheelchairs.

• Delivery required no later than 7/31/2026.

• Vendor must provide Notification of Shipment, Carrier, and Tracking Number to the VA COR POC.

• Shipments must be labeled with the Purchase Order number.

• Serial Numbers (when applicable) must be provided.

• Vendor must adhere to VA San Diego COVID-19 rules and use PPE on campus.

• Delivery Deadline: July 31, 2026.

• Place of Delivery: SCI/CLC Building, Room 1B126, VA San Diego Healthcare System, 3350 La Jolla Village Dr., San Diego, CA 92161.

• Delivery requires a liftgate or ramp and moving equipment; no loading dock is available.

• Warranty: Standard manufacturer industry warranty term required. Extended warranties requiring advance payment are not accepted.

• Security: Vendor must comply with VA information and information system security standards (FISMA, NIST, VAAR 852.273-75). Personnel requiring access to VA computer systems must undergo a background investigation and receive favorable adjudication per VA Directive 0710.

• Payment: Upon receipt of a properly prepared invoice referencing the contract/PO number and correct payment address.

• Non-contract Charges: Require prior approval from the Contracting Officer.
