Statement of Work (SOW) / Performance Work Statement (PWS)

To procure specialized physical therapy equipment for wheelchair-bound patients at the VA San Diego Spinal Cord Injury (SCI) and Community Living Center (CLC).

This SOW requires contractors to provide specific, brand-name-only physical therapy equipment that meets or exceeds the listed salient characteristics. The equipment is essential for rehabilitative therapy services for patients in wheelchairs.

Key equipment required includes:

• Multi-Station Weight Training Apparatus (Equalizer Legacy 1000 5 Station Gym, Model #1005VB): Must be wheelchair accessible, featuring vertical bench press, rowing, seated overhead press, 2 Hi/Lo pulleys (2-50 lb stacks), lat pull down (200 lb stack), and vertical butterfly (200 lb stack). • Dip Rickshaw (Equalizer, Model #FM-9223-78794): Must accommodate standard and powered wheelchairs, feature an integrated 300lb weight stack, and allow customizable resistance. Approximate size: 45” W x 71.25” D x 96”H. Quantity: 1.

• Equipment must meet or exceed specified physical, functional, and performance characteristics.

• Standard manufacturer industry warranty term required; extended warranties requiring advance payment are not accepted.

• Delivery Deadline: No later than July 31, 2026.

• Place of Delivery: SCI/CLC Building, Room 1B126, VA San Diego Healthcare System, 3350 La Jolla Village Dr., San Diego, CA 92161.

• Delivery requires coordination with the VA COR POC. No loading dock is available; delivery trucks must have a liftgate or ramp.

• Brand Name Only: Equipment must meet or exceed the salient characteristics of the specified brand name items.

• Delivery Coordination: Vendors must notify the VA COR POC of shipment, provide carrier and tracking information, and label shipments with the Purchase Order number.

• COVID-19 Protocols: Vendors must adhere to VA San Diego's COVID-19 rules and use personal protective equipment (PPE) on campus.

• Security: Vendors must comply with VA information and IT security standards, including background investigations for personnel accessing VA computer systems per VA Directive 0710.

• Payment: Made upon receipt of a properly prepared invoice referencing the contract/PO number and correct payment address.

• Shipping: All shipping costs for parts are included in the contract cost.
