Statement of Work (SOW) / Performance Work Statement (PWS)

To replace end-of-life dental equipment at the Edith Nourse Rogers Memorial Veterans Hospital, Bedford VA Medical Center, to prevent shortages of essential dental handpieces and ensure compatibility with updated motors.

• Provide labor, personnel, equipment, tools, materials, and supervision for the installation and de-installation of dental equipment.

• Specific equipment to be provided includes:

  • A-Dec 532PRO TRAD Radius Delivery Systems (6)
  • A-Dec 545STA 12 O'Clock Assistant Instrumentation Units (6)
  • A-Dec 541PRO 12 O'Clock DUO Delivery System (1)
  • A-Dec Doctor's Stools (7)
  • A-Dec Assistant's Stools (7)
  • Kavo INTRA LUX KL 703 LED handpiece motors (108)
  • Kavo M05L Mini handpieces (72)
  • Kavo E10C handpieces (72)
  • Kavo E20L handpieces (36)

• Installation of all listed new equipment.

• De-installation of all current equipment being replaced.

• Location: Edith Nourse Rogers Memorial Veterans Hospital, Bedford VA Medical Center, 200 Springs Rd, Bedford, MA 01730.

• Work is to be completed in a single visit (single or multiple days as required).

• Installation dates to be coordinated with the Bedford VA Medical Center's Healthcare Technology Management service.

• All personnel must comply with VA security management programs and obtain VA police permission.

• Installation personnel must be manufacturer-trained; resumes and certifications required prior to installation.

• Contractor to communicate estimated noise levels during installation and modify methods or timing to stay within acceptable thresholds.

• Contractor responsible for safe removal and disposal of all packing materials and waste.

• Comprehensive onsite training program for operating clinicians, including technical, operational, administrative, and troubleshooting sessions. Training to occur during normal business hours (8:00 am - 4:30 pm, Monday-Friday).

• Compliance with Infection Control Risk Assessment requirements.

• Compliance with security, privacy, and safety issues according to VA standards.

• Contractor responsible for preventing and reporting any damage to government property.

• Employees must wear visible identification, park in designated areas, and adhere to VA regulations regarding smoking, cellular phone use (within six feet of medical equipment), and weapons.

• Compliance with OSHA Bloodborne Pathogens Standard, including education, policies, and provision of appropriate personal protective equipment.

• Adherence to specific Records Management Language for Statement of Work (SOW) requirements, including compliance with relevant laws, regulations, and agency directives regarding data handling, ownership, and destruction.

• Strict adherence to VA information security and custodial language, including data protection, sanitization, and restrictions on use and disclosure of VA information.
