• Representation Section: Requires the offeror to check a box indicating whether they represent that their offer does or does not meet the definition of an "Indian Economic Enterprise" with "Indian Ownership."
• Information Fields: Includes spaces for "Name of Federally Recognized Tribal Entity," "Name of the 51% owner of IEE Firm," "Business Name," "UEI Number," and "Certifying Signature" (with fields for Print Name).

Solicitation / RFP / RFQ

This is a combined synopsis and solicitation (RFQ) for commercial items, specifically network equipment for the Crownpoint Service Unit. • Delivery is required within 60 days After Receipt of Order (ARO).

• The period of performance for the overall project is May to June 2025.

• Offers are due by April 20, 2026, at 10:00 AM MDT.

• Submission location: Crownpoint, NM 87313.

• Evaluation criteria: Lowest Price Technically Acceptable (LPTA).
