Scope of Work

• Construction of a new commercial driveway approach, approximately 30-ft wide, with ADA-compliant truncated domes.
• Installation of approximately 143-160 linear feet of 6-ft tall chain link fencing with 3-string barbwire.
• Installation of two chain link swinging gates for a 30-ft clear opening, also with barbwire.
• Obtaining necessary permits, including an Encroachment Permit from the City of San Dimas.
• Traffic control, surveying, and materials/compaction testing.
• Final cleanup. The work will be performed at 444 E. Bonita Ave, San Dimas, CA, including portions within the City of San Dimas right-of-way. Visual details are provided in Attachment 4 (SOW Exhibits).

• Evaluation Criteria: Lowest Price Technically Acceptable (LPTA). Technical acceptability will be rated "acceptable" or "unacceptable," and Past Performance will be rated "Acceptable" or "Neutral." Offerors must provide an explanation of their ability to perform.

• Contractor must provide two weeks' advance written notification to affected residents/businesses.
• Key personnel requirements include Project Manager, Estimator, Site Superintendent, Foreman, and Quality Control Inspector.
• Deliverables include Project Plan, Construction Schedule, Safety Plan, QC Plan, and As-Built Drawings.
