• References: Credit and banking references (contact information only, no account details).
• Remarks: A section for additional relevant information.
• Certification: A statement by the bidder certifying the accuracy of the provided information.

Bidders must complete this questionnaire thoroughly and accurately to demonstrate their relevant experience and qualifications. The information provided will be a critical factor in the Government's evaluation of the bidder's responsibility and capability to successfully perform the contract. Failure to provide sufficient detail may hinder the evaluation process.

The objective is to mitigate safety hazards on roads and recreation sites by delivering various sizes of stone to predetermined locations within the USFS Eastern Divide RD roads department.

• Deliver or pick up various sizes of stones to specified locations across the District. A list of roads with GPS coordinates is attached.
• Close coordination with USFS roads staff is required due to the dynamic nature of stone size and location needs.
• Required stone sizes include: Crusher Run, Pug Mill, #57, #357, #1, #10, Erosion Control Stone, and Rip Rap Class 1.

April 10th, 2025 – April 10th, 2026

• Total available funding is $60,000.
• Costs are highly dependent on the selected quarry's proximity to the required locations.

Solicitation / RFP / RFQ

This document is a combined synopsis/solicitation for commercial items, serving as the sole solicitation. It is a Request for Quotation (RFQ) for the delivery or pickup of various types of stone to predetermined locations. The acquisition is a Total Small Business Set-Aside.

• Delivery or pickup of various sizes of stone to predetermined locations across the district.

• Two distinct periods of performance are outlined:

  • 04/13/2026 to 04/12/2027
  • 04/14/2026 to 04/13/2027

• Anticipated contract type: Firm-fixed price requirements contract.

• The government intends to make one award.

• Quotations must include pricing for all items to be considered responsive.

• Evaluation will be based on the following criteria:

  • Technical Approach: Ability to meet requirements and understanding of the SOW.
  • Price: Fair and reasonable, evaluated in accordance with FAR 12.204.
  • Past Performance: Assessed as acceptable or neutral.

• Award will be made to the offeror representing the best value, determined by the highest technically rated offeror whose price is fair and reasonable and whose past performance is acceptable or neutral. Tradeoffs will be conducted.

• Offerors must hold prices firm for 90 calendar days from the offer due date.

• This acquisition is set-aside for small business concerns.
