This document is a "Business Management Questionnaire" required for offerors responding to Solicitation Number CT2386-26 Air Force Fabric. Its purpose is to collect recent and relevant information (within the last 3 years) about offerors' past performance on contracts and subcontracts to demonstrate their ability to perform the proposed effort.

• Contract Information: Contract Number, Contractor Name/Address/Contact, Type of Contract, Contract Dollar Value, Date of Award, Date Completed.
• Performance Details: Type/Extent of Subcontracting, Complexity of Product/Service, Percentage of work completed by the offeror's company.
• Work Description: Details of supply/service provided, location, company name, and relevancy of the work.
• Reference Contact: Name, Address, Telephone Number, Email of a contact person who can speak to the offeror's performance on the referenced contract.

Offerors must complete this form for each relevant contract or subcontract reference (up to 3 years old) to demonstrate their capability and past performance. The form may be duplicated if multiple references are needed. This information will be used to assess the bidder's ability to perform the proposed effort for the Air Force Fabric solicitation.

It collects essential company, banking, and registration information necessary for payment processing and vendor management. Bidders should ensure their SAM.gov profile is current and be prepared to provide accurate banking details.

Solicitation / RFP / RFQ

This is a combined synopsis/solicitation for commercial items, issued as a Request for Quote (RFQ) under solicitation number CT2386-26. UNICOR, Federal Prison Industries, Inc. intends to procure fabric for clothing.

• The solicitation is for a five-year fixed-price indefinite delivery-indefinite quantity contract.

• Two types of fabric are required:

  • Item 00001: FABAFSPM (Herringbone fabric, 65% Polyester/35% Cotton, 4.25-4.75 oz/sq yd, blue color, 60-62" width).
  • Item 00002: FAB0625 (100% Polyester Micro Denier, black color, 2.7+/-0.3 oz/sq yd, minimum 60" width).

• Fabric must be Berry Amendment compliant and domestic USA only. No foreign items or components are acceptable.

• Material must comply with DFARS 252.225-7012, Preference for Certain Domestic Commodities.

• Non-manufacturer rule applies.

• A Certificate of Conformance and applicable physical test results must accompany each shipment.

• Contract Type: Five-year fixed-price indefinite delivery-indefinite quantity.

• Delivery: FOB Destination to UNICOR, Butner Cutting Operation-FMC, Butner, NC.

• UNICOR's desired delivery is 45 days or sooner after issuance of each delivery order.

• Offer Due Date: Thursday, May 21, 2026, at 2:00 PM CST.

• Proposals must be submitted via email to adam.mccammack2@usdoj.gov. Offerors are responsible for confirming receipt.

• Offerors must hold prices firm for 60 calendar days from the proposal closing date.

• Evaluation Factors: Past performance and technical factors are significantly more important than price. Technical factors include adherence to Berry Amendment compliant material. Past performance factors include quality, delivery, shipping instructions, customer service, and performance within the last three years.

• The government may award multiple contracts.

• This requirement is 100% set-aside for small business.

• NAICS Code: 313210.

• Small Business Size Standard: 1,000 Employees.

• This is a DO-C9 rated order.

• Quantities are estimates only. Guaranteed minimum quantities are 72,000 yds for FAB0625 and 50,000 yds for FABAFSPM. Estimated/Not-to-Exceed quantities are 720,000 yds for FAB0625 and 500,000 yds for FABAFSPM.

• Standard Form 1449, Amendments, Business Management Questionnaires, ACH form (if new to UNICOR), Credit Release Form, Memo certifying Berry Amendment compliance, and Small Business Program Representations are required.
