Statement of Work (SOW) / Performance Work Statement (PWS)

To provide qualified Basic Life Support (BLS) and Advance Life Support (ALS) ambulance services on an as-needed basis for the inmate population at the Federal Correctional Complex (FCC) in Pollock, Louisiana.

• Provide ambulance transport services for inmates experiencing medical emergencies (unscheduled) and for non-emergency medical appointments (scheduled).

• Services include BLS and ALS, requiring all necessary supplies, equipment, staff, licensing, and expertise.

• Estimated volume: approximately 4 ambulance service calls per week (16 per month), with the understanding that unforeseen situations may require additional services.

• Trips include drop-off and/or pick-up of inmates.

• Scheduled transports require a minimum of 48 hours' notice.

• Response time for unscheduled emergency transport: no more than 15 minutes from the initial request, 24/7.

• Response time for emergency transports: within 15 minutes of the call.

• Contractor must maintain open communication and report delays immediately.

• A minimum of 75% of all scheduled and unscheduled ALS/BLS trips will be provided by the contracted company.

• Monthly summary report to the Contracting Officer Representative (COR) detailing calls requested, calls taken/responded to, and calls deferred to local EMS.

• Services are required at FCC Pollock, located at 1000 Airbase Road, Pollock, LA 71467.

• Transport destinations include Rapides Regional Medical Center, Christus St. Frances Cabrini Hospital, and Riverside Hospital in Alexandria, LA, as well as other locations as required by FCC Pollock.

• Contractor must hold a current State of Louisiana license for BLS and ALS ambulance services.

• Compliance with Federal, State, and City codes, and DHH 105 CMR 170.305 (c) regulations.

• Staffing requirements include specific configurations of Paramedic, EMT-Intermediate, Advanced EMT, or EMT-Basic.

• All personnel must have appropriate educational experience and training.

• Contractor must provide all necessary medical supplies and equipment.

• Contractor must be certified, licensed, or officially recognized by the relevant government entities.

• Contractor must provide insurance certificates.

• Contractor must be registered in the System for Award Management (SAM).gov.

• Contractor must develop a contingency plan for catastrophic incidents.

• Security procedures mandated by FCC Pollock and the Federal Bureau of Prisons must be followed, including escorting of ambulances and potential inspection of equipment.

• A staff member from FCC Pollock may ride with the inmate.

• Payment is based on a fixed/capitated price per transport, with mileage limitations and specific billing procedures.

• Contractor must be the authorized emergency service provider for Grant Parish.
