Statement of Work (SOW) / Performance Work Statement (PWS)

To perform annual inspections and load testing of five (5) overhead hoists at the 158th Fighter Wing, Vermont Air National Guard, to ensure compliance with Department of the Air Force Manual (DAFMAN) 91-203 and relevant safety standards.

• Conduct annual safety inspections and load tests for five designated overhead hoists.

• Provide detailed inspection reports and load test certifications for each hoist.

• Submit itemized quotes for any noted deficiencies within 15 days.

• Immediately inform the Contracting Officer's Representative (COR) of any safety hazards.

• Dispatch technicians within five business days to troubleshoot and repair inoperable hoists.

• Submit repair estimates for COR review before performing repairs.

• Complete repairs within 10 business days of contract modification issuance.

• Inspections and load tests must meet standards outlined in DAFMAN 91-203, OSHA Regulations, and ASME standards.

• Load tests must be performed at 100-125% of the rated load.

• No more than two instances of delayed scheduling for repair services.

• No more than one instance of delayed completion for repairs.

• Inspection reports to be provided within 15 calendar days following inspection.

• Load test certificates to be provided within 15 calendar days following successful testing.

• Period of Performance: 1 Base Year of 12 months and 2 12-month option years.

• Place of Performance: Multiple facilities generally located around 112 NCO Drive in South Burlington, Vermont.

• Service Hours: Tuesday through Friday, 0700 - 1600, excluding recognized US holidays or facility closures.

• Contractor employees must possess all required licenses for operating vehicles and equipment.

• Employees must complete Antiterrorism Awareness Level I Training within 15 calendar days of contract start.

• Contractor personnel must identify themselves as contractors and wear distinguishing badges.

• Compliance with all installation security, traffic, and safety regulations is required.

• No weapons, contraband, or unauthorized substances are permitted on the installation.

• Contractor must designate a Contract Manager (CM) with full authority to act on contract matters.

• Adherence to a zero-tolerance policy regarding trafficking in persons.

• Management of Organizational Conflicts of Interest (OCI) is required.

• The Government intends to award a single Firm-Fixed Price (FFP) Contract.

• Submission Due Date: 22 May 2026 at 03:00 PM EDT.

• Acceptable Formats: MS Office 2013 compatible (MS Word, MS Excel). PDF is acceptable for Technical Capabilities and Past Performance. Price Quote must be in MS Excel.

• Quote Acceptance Period: Must be valid until 30 September 2026.

• Evaluation Factors: Technical Capability, Past Performance, and Price.

• Technical Capability Rating: "acceptable" or "not acceptable".

• Past Performance Rating: "acceptable" or "not acceptable". "Unknown" past performance is considered "acceptable".

• Price Evaluation: Will assess reasonableness, completeness, and absence of unbalanced pricing using techniques in FAR 13.106-3.

• Offerors shall scan all files for computer viruses prior to submission.

• The Government will base its evaluation on the information presented in the offeror’s Quote.
