This document, titled "Supplemental Offer Information," is a form designed to collect essential company and relevant work history details from potential bidders. It aims to assess a bidder's experience and capability to perform work similar in scope and size to the acquisition requirements.

• Company Information: Requires the Name of Firm, Address of Firm, Phone Number, and Contact Person Name.
• Relevant Work History: Bidders must list at least three projects completed within the last 5 years that are of similar size and scope to the current acquisition. This section is divided into:
  • Past Work for the FAA
  • Past Work for other US Government agencies
  • Past Work of a similar scope and cost performance for Others For each project, bidders need to provide the Year, Owner, Location, a Detail description of Work, and the Amount.
• List References: Bidders must provide references for three projects from the above lists that are most similar in scope and cost to the requirement. This includes Year, Owner, Location, and REFERENCE details (Contact Name, Phone Number, and email).

This form is crucial for bidders to demonstrate their qualifications and experience. Completing it accurately and thoroughly will help the government evaluate a bidder's ability to successfully execute the contract. The information provided here will likely be a key component of the technical or past performance evaluation.

Statement of Work (SOW)

To clear trees and tree limbs obstructing the line of sight for L3Harris Technology's Microwave data path at the QRV ATCBI facility, located on the Justin P. Wilson Cumberland Trail State Park. The FAA has a lease agreement allowing clearing up to 1000 feet from the leased property.

• Cut tree limbs and, if necessary, remove trees that obstruct the microwave data path up to 1000 feet from the FAA leased property.

• Clear fallen material from roads or trails that could cause trip hazards or blockages.

• Mulch and dispose of cut limbs, brush, and small debris in an approved location.

• Cut logs into salvageable lengths and place them in an approved area.

• Individual trees to be trimmed or removed will be categorized by diameter at breast height (dbh): <12" dbh and 12-24" dbh. Approximately 25 trees from each category are anticipated.

• Removal of vegetation 3 inches or less in diameter is considered incidental.

• Fell trees not expected to survive extensive trimming and grind stumps to below ground level.

• Protect underground or overhead utilities.

• Work must ensure the microwave path is completely clear of tree obstructions.

• Work is to be performed under the supervision of a licensed arborist in Tennessee, with an arborist onsite during trimming and removal.

• Trees to be affected must be identified and marked for review by a State of Tennessee representative within 30 days of receiving the NTP.

• Work must occur between October 1st and March 31st to avoid negative impacts on critical species.

• Work hours are Monday through Friday, 0600 AM to 1630 PM, unless otherwise coordinated.

• Location: QRV ATCBI, 3405 Owl Roost Rd, Crab Orchard, TN 37723.

• Contractor must coordinate all work with the FAA COR and L3Harris representative.

• Contractor must protect underground or overhead utilities.

• Contractor must develop and implement a site-specific comprehensive Health and Safety Plan (HASP).

• Contractor must comply with OSHA regulations.

• Contractor is responsible for all supplies and equipment.

• Contractor must secure all work areas at the end of each workday.

• Pre-bid site visit is recommended; all expenses are the responsibility of the contractor.
