To ensure all backflow prevention assemblies at USCG Base Ketchikan are functioning correctly, are certified in accordance with applicable standards, and that any deficiencies are documented and reported for corrective action.

• Provide annual inspection, testing, and certification for all backflow prevention assemblies listed.

• Tasks include inspection, testing, certification, and reporting.

• A list of 23 specific backflow preventer assemblies at various locations within USCG Base Ketchikan is provided, including brand, size, and location.

• For any assembly failing inspection or found deficient, provide a written quote for necessary repairs.

• All 23 backflow preventers must be inspected and tested annually per applicable standards.

• A final report must be submitted within 15 days of work completion, including verified equipment information, list of devices tested, identified code violations/deficiencies, and technician-signed forms for each device.

• Quotes for deficiencies must be provided with the final report.

• Proof of technician certification/license must be provided upon request and included with the final report.

• Period of Performance: Base Period (July 1, 2026 - September 30, 2026) with four (4) one-year option periods through September 30, 2030.

• Place of Performance: USCG Base Ketchikan, 1300 Stedman Street, Ketchikan, AK 99901.

• Hours of Operation: Normal base work hours (0730-1700 Mon-Thu, 0730-1530 Fri), excluding Federal holidays, unless prior approval is obtained for off-hours work.

• A Government escort will be provided for all on-site work.
