Statement of Work (SOW) / Performance Work Statement (PWS)

Amendment / Modification

This document is an Addendum to a Blanket Purchase Agreement (BPA). It outlines the terms and conditions for a decentralized BPA.

Each order will be assigned a call number in the format "HT0014-2X-FXXX", where the last four numbers are system-assigned numeric characters and the fiscal year number updates accordingly.

The BPA will be administered by the Healthcare Contracting Division – Northeast (HCD-NE). Only the Contracting Officer (CO) is authorized to approve changes or modify requirements. All contractual problems must be referred to the CO.
