This announcement is a Request for Quote (RFQ) for Firm-Fixed Price contract services to move furniture and equipment from Buildings 7233 and 4230 to 6015. This is the only solicitation, and a written solicitation will not be issued.

Statement of Work (SOW)

To provide all labor, equipment, tools, packing material, supervision, and other necessary items to move government equipment from Building 7233 and Building 4230 to Building 6015 at Dyess AFB, TX. This move is to facilitate the relocation to a renovated Building 6015.

• Mobilize manpower and equipment to Dyess Air Force Base.

• Check-in at the Tye Gate and proceed to Buildings 7233 and 4230.

• Move specified items including cabinets, chairs, couches, credenzas, desks, lamps, microwaves, monitors, printers, refrigerators, safes, shelves, shredders, a large metal cabinet, portable A/C units, standing desks, tables, time zone clocks, TVs, wall art, fixtures, area rugs, and boxes of office supplies.

• Provide 75 large boxes (18"x18"x24"), 75 medium boxes (18"x18"x16"), paper, and tape for 150 boxes.

• Secure load and transport all furniture and equipment to Building 6015, located approximately 0.5 miles away.

• Buildings 7233 and 6015 are two-story with multiple stairwells. Building 6015 has an elevator but no loading dock. Building 7233 has no elevator and no loading dock. Building 4230 is a one-story building with no loading dock.

• Lay appropriate protective materials to protect existing building, interim building, and asphalt.

• Remove and clean any debris and refuse left upon completion of the move.

• The contractor shall have five (5) days to complete all work associated with the contract. Day 1 begins on the date of supplies delivered. Day 3 is for packing by building occupants. Day 4 and 5 are the contractor's move dates.

• Safety: All holes/pits/trenches/manway openings must be surrounded by a 48-inch high mesh fence with visible orange coating. Holes must be covered with three-quarter inch plywood or metal grating when not being worked on. Adherence to OSHA requirements and Texas Workman Compensation laws is mandatory.

• Welding/Cutting: Obtain required advance approval (AF Form 592) 24 hours in advance for any process involving flames or high heat. A fire watch and appropriate fire extinguishers must be on-site.

• Confined Space: Obtain all necessary approvals and permits before entering confined spaces.

• Radiation: Contractors using devices with radioactive materials must obtain written authorization/permit from the Dyess AFB Radiation Safety Officer at least 45 days prior to use.

• Security: Dyess AFB is a closed base. All personnel require base identification and vehicle passes. A Dyess Form 173 must be completed for entry. Trusted Agent Letters are required for day laborers.

• Vehicles: Contractor vehicles must be marked with company name. All employees driving on base must have a valid driver's license, registration, and insurance. Heavy equipment operators must be licensed/certified.

• Access: Obtain base identification and vehicle passes for all personnel. Submit a written request to the contracting officer. Provide an after-hours contact number for EMNS.

• Hazardous Conditions: Immediately notify the Contracting Officer if conditions prevent task performance.

• Waste Disposal: Title to waste, excess, scrap, and salvage materials is vested in the Contractor, unless specifically excepted. Materials must be disposed of off-base in accordance with applicable regulations.

• Reasonable utilities and workspace.

• Access to existing toilet facilities in the immediate work area.

• General on-base security police and fire protection service.

• No telephone services will be provided by the government. Contractor may use government phones for emergencies only (no long distance calls).

• Upon completion, notify the Government Representative (GR) for a final joint inspection and walk-through.

• Failure to comply with requirements may result in withholding of final payment.
