This document is the "Indian Health Service Buy Indian Act Indian Economic Enterprise Representation Form." • Eligibility Requirements: The enterprise must meet the definition of an "Indian Economic Enterprise" at the time of offer, contract award, and throughout the contract performance period. Failure to meet these requirements necessitates immediate written notification to the Contracting Officer.
• Documentation: Contracting Officers may request documentation of eligibility before awarding contracts under the Buy Indian Act. Awards may be subject to protest if eligibility is in question.
• Registration: Successful Offerors must be registered with the System of Award Management (SAM).
• Penalties: Submission of false or misleading information is a violation of law and punishable under 18 U.S.C. 1001. False claims are subject to penalties enumerated in 31 U.S.C. 3729 to 3731 and 18 U.S.C. 287.
• Representation Section: Offerors must indicate whether they do or do not meet the definition of an "Indian Economic Enterprise" with "Indian Ownership."
• Required Fields: Name of Federally Recognized Tribal Entity, Name of the 51% owner of IEE Firm, Business Name, DUNS Number, Certifying Signature, and Print Name.

This form is critical for any bidder seeking to qualify for contracts set aside or awarded under the Buy Indian Act. Bidders must accurately complete this form to self-certify their eligibility and provide the required business information. Failure to meet the criteria or accurately complete the form could impact their ability to be considered for or awarded the contract.

Solicitation / RFP / RFQ

This is a combined synopsis/solicitation (RFQ) for Boiler, Water Heater, and Chiller preventive maintenance and repair services for the Albuquerque Area Indian Health Service (AAIHS). The services are required for facilities at the Santa Fe Service Unit and the Taos-Picuris Service Unit. This acquisition is a total Indian Small Business Economic Enterprise (ISBEE) Set-Aside, with a cascading set-aside for other small businesses if competition is insufficient.

• Preventive maintenance (PM) and minor repairs for boilers, water heaters, chillers, and pumps.

• Services to be performed in accordance with manufacturer recommendations and applicable codes.

• Specific equipment covered includes Camus Boilers, Noritz Tankless Water Heaters, Chiller Systems, Cooling Tower Pump Systems, and associated pump systems.

• Deliverables include inspection reports, parts replaced, combustion analysis documentation, deficiency reports, and service tickets.

• Emergency service response time not to exceed four (4) hours.

• Contract Type: Firm-fixed price Purchase Order (PO).

• Period of Performance (Base Year): TBD/2026 – TBD/2027.

• Options: The Government may extend services for up to 6 months, and the total contract duration may not exceed five years.

• Offer Due Date: May 19, 2026, 12:30 PM MT.

• Submission Requirements: A complete quotation package including Technical Qualifications, Past Performance, and Price, along with a signed SF-1449 and IHS Buy Indian Act Indian Economic Enterprise Representation Form.

• Evaluation Factors (in descending order of importance): Technical Capability (experience, personnel, response times), Past Performance (recency, relevance), and Price.

• Award will be made to the responsible quoter offering the best value.

• Total Indian Small Business Economic Enterprise (ISBEE) Set-Aside.

• Cascading set-aside for other Small Business concerns.
