This document is the "Indian Health Service Buy Indian Act Indian Economic Enterprise Representation Form." It serves as a self-certification for offerors to confirm they meet the definition of an "Indian Economic Enterprise" as required by the Buy Indian Act. This form is to be submitted in response to a Sources Sought Notice, Request for Information (RFI), or Solicitation.

• Self-Certification: Offerors must attest whether they meet the definition of an "Indian Economic Enterprise" with "Indian Ownership" under the Buy Indian Act.
• Eligibility Period: The enterprise must meet the definition throughout the offer period, at contract award, and during the full contract performance period. Immediate notification is required if eligibility is lost.
• Required Information: The form requires the Offeror to provide:
  • Name of Federally Recognized Tribal Entity (if applicable)
  • Name of the 51% owner of the IEE Firm
  • Business Name
  • Unique Entity Identifier (UEI) Number
  • Certifying Signature and Printed Name
• Legal Consequences: False or misleading information is a violation of law, punishable under 18 U.S.C. 1001, and false claims are subject to penalties under 31 U.S.C. 3729-3731 and 18 U.S.C. 287.
• Additional Requirements: Successful offerors must also be registered with the System of Award Management (SAM).

This form is critical for any bidder seeking to be considered for contracts or awards under the Buy Indian Act. Bidders must accurately complete and submit this representation to self-certify their eligibility. Failure to meet the definition or provide accurate information can lead to disqualification or penalties. It also highlights the need for SAM registration.

Statement of Work (SOW) / Performance Work Statement (PWS)

To obtain professional radio broadcasting services to disseminate public health information, advance Navajo Area public health priorities, promote health education, and strengthen community engagement through accessible and culturally relevant programming.

• Air a minimum of five (5) distinct thirty-second (30-second) public service announcements (PSAs) during the period of performance.

• Broadcast each approved PSA three (3) times per day, seven (7) days per week, for a continuous five-month period.

• Provide airtime for PSAs approved by the NAO HPDP Health Communication Team.

• Provide consultation and technical assistance on PSA scheduling and broadcasting.

• Coordinate with NAO HPDP to plan and schedule PSA broadcasts.

• Develop and submit a detailed broadcast schedule for Government review and approval.

• Broadcast PSAs on approved designated stations.

• Detailed broadcast schedule for approval prior to airing.

• Monthly airing logs documenting PSA broadcasts.

• Monthly performance reports summarizing broadcast activities.

• A final report summarizing total broadcasts and overall campaign implementation.

• Documentation of coordination and planning activities, as requested.

• Accurate and timely invoices.

July 1, 2026, through November 30, 2026 (five months).

• Contractor shall provide all personnel, equipment, facilities, and materials.
• Government will provide all necessary information.
• All deliverables shall be submitted electronically to the COR on a monthly basis unless otherwise specified.

Sources Sought Notice

This notice is for market research purposes only and is not a solicitation. The Navajo Area Indian Health Service (NAIHS) is seeking to identify qualified Indian Economic Enterprises (IEEs) and Indian Small Business Economic Enterprises (ISBEEs) capable of providing radio broadcasting services. The goal is to disseminate vital public health information to communities within the Navajo Area and surrounding regions. The information gathered will help determine the acquisition strategy, with a priority for a set-aside under the Buy Indian Act.

The contractor will be required to broadcast Government-approved Public Service Announcements (PSAs). This includes:

• Broadcasting five (5) distinct 30-second PSAs, three times per day, seven days a week.

• Providing consultation on PSA scheduling for maximum audience reach.

• Developing and submitting a detailed broadcast schedule for Government approval.

• Broadcasting approved PSAs on designated radio stations.

• Detailed broadcast schedule for approval.

• Monthly airing logs.

• Monthly performance reports.

• Final summary report.

Anticipated: July 1, 2026, through November 30, 2026 (five months).

Navajo Area and surrounding regions. Contractor administrative functions may be performed at their own facilities.

Interested parties should submit a capability statement (max 3 pages) demonstrating their ability to perform the services. The statement should include:

• Company Information (Name, address, DUNS/UEI, POC).
• Business Size and Status (under NAICS 516110).
• Capability Narrative (experience in radio broadcasting, public health campaigns, rural/tribal communities).
• Coverage Area (list of radio stations and geographic coverage relevant to the Navajo Nation).
