This document is a Business Management Questionnaire for Solicitation Number CT2387-26. It is designed to collect recent and relevant information about offerors' past performance on contracts and subcontracts (Federal, State, local government, or private) within the last three years. This information will demonstrate the offeror's ability to perform the proposed effort.

• Contract Number: Field to enter the contract number.
• Contractor Information: Name, Address, Zip Code, Telephone Number, and Email Address of the contractor.
• Type of Contract: To specify the type of contract.
• Contract Dollar Value: To indicate the value of the contract.
• Date of Award: The date the contract was awarded.
• Date Completed: The date the contract was completed (or status if not completed).
• Type/Extent of Subcontracting: Information on subcontracting activities.
• Complexity of Product/Service: Description of the complexity involved.
• Percentage of work completed by your company: To quantify the offeror's direct involvement.
• Description of supply/service(s) provided: Details on the nature of the work, location, company name, and its relevancy.
• Contact Person Information: Name, Address, Telephone Number, Email, position, and company name of a contact person who can speak to the offeror's performance on the contract.

Offerors must complete this questionnaire to provide evidence of their past performance and capability. The information submitted will be used to assess their ability to successfully execute the requirements of Solicitation Number CT2387-26. Each contract reference requires a separate form, which may be duplicated.

This document is an ACH Vendor/Miscellaneous Payment Enrollment Form used by UNICOR, Federal Prison Industries, Inc. It is for automated clearing house (ACH) payments with an addendum record processed through the Vendor Express Program. Recipients of these payments should provide this information to their financial institution when presenting this form for completion.

• Agency Information: Federal Program Agency, Agency Identifier, Agency Location Code, ACH Format, Mailing Address, Vendor Payment Contact (Name, Telephone, Fax, Email).
• Payee/Company Information: Company Name, Company Address, DUN & Bradstreet Number, Current Date, Contact Person (Name, Telephone, Email, Fax), Taxpayer Identification No (TIN) or SSN No, Additional Information.
• Financial Institution Information: Name, Address, ACH Coordinator Name, Telephone Number, Nine Digit Routing Transit Number, Depositor Account Title, Depositor Account Number, Lockbox Number, Type of Account (Checking, Savings, Lockbox).
• Authorization: Signature and Title of Authorized Official, Telephone.

Vendors who are awarded contracts with UNICOR and are to be paid via ACH through the Vendor Express Program will likely need to complete this form to provide their banking and payment information for direct deposit of funds.

Solicitation / RFP / RFQ

This is a combined synopsis/solicitation for commercial items, issued as a Request for Quotation (RFQ) under solicitation number CT2387-26. UNICOR, Federal Prison Industries, Inc., intends to award an indefinite delivery-indefinite quantity (IDIQ) contract, with a base period and four option years, for various buttons. The North American Industry Classification System (NAICS) code is 339993, with a size standard of 750 employees. This acquisition is a small business set-aside.

• The contract will be for various types of buttons, as detailed in Section B.

• Fabric must be Berry Amendment compliant and domestic USA only. No foreign items or components are acceptable.

• Material must comply with DFARS 252.225-7012, Preference for Certain Domestic Commodities.

• The Non-Manufacturer Rule applies.

• A Certificate of Conformance (COC), applicable physical test results, and packing slip must accompany each shipment.

• Guaranteed minimum quantity is 1,000 ea. Estimated quantity/not-to-exceed quantity for the life of the contract is 38,000,000 ea.

• This is a Rated Order DO-C9 requirement.

• UNICOR's desired delivery is 45 days or sooner after issuance of each delivery order.

• The contract type is an indefinite delivery-indefinite quantity (IDIQ).

• The offer period is 60 days.

• Offer Due Date: Friday, June 12, 2026, at 2:00 PM CST.

• Submission Method: Emailed proposals are accepted. Offerors are responsible for confirming receipt.

• Questions Deadline: June 5, 2026. Questions must be submitted in writing via email only.

• Evaluation Factors: Past performance and technical factors are significantly more important than price. Technical factors include adherence to Berry Amendment compliant material. Past performance factors include quality of item supplied, on-time delivery, adherence to shipping instructions, customer service/business relations performance, and performance within the last three years.

• This requirement is a small business set-aside.

• All contractors must register in SAM.gov prior to award.

• Offerors must hold prices firm for 60 calendar days from the date specified for receipt of offers.

• The contract will be administered by Federal Prison Industries, Central Office, Washington, DC.
