Section 2: Evaluator Information - To be completed by the Client/Evaluator, including:

• Evaluator Name, Position, Address, Phone, Email
• Role in the Project
• Ratings on Quality, Schedule/Timeliness, Communication, Management/Personnel/Labor, Cost/Financial Management, and Subcontract Management.
• A summary question: "Would you hire or work with this firm again?"
• Adjectival ratings definitions (Outstanding, Above Average, Satisfactory, Unsatisfactory, Not Applicable) are provided for guidance.

Offerors must provide this questionnaire to their clients to complete and return. The completed PPQ is to be incorporated into the Offeror's SF330 submission. Bidders should ensure their clients understand the importance of thoroughly completing the questionnaire, as it directly impacts the evaluation of their past performance. The solicitation's due date for the SF330 submission should be referenced for timely completion.

Solicitation / RFP / RFQ

This is a Request for Standard Form (SF) 330 Architect-Engineer Qualification Packages for an on-ramping opportunity to the Veterans Health Administration (VHA) Program Contracting Activity Central (PCAC) Indefinite-Delivery, Indefinite-Quantity (IDIQ) Multiple Award Task Order Contract (MATOC) for Architect-Engineering (A-E) services. Services include developing complete construction documents, working drawings, specifications, reports, and construction period services for multiple projects at VA medical facilities nationwide. This is not a Request for Proposal; no solicitation package will be issued. The government is not obligated to award a contract.

Questions & Answers / Clarifications

This document provides responses to vendor questions regarding the PCAC A-E IDIQ MATOC solicitation, clarifying submission requirements and evaluation criteria.

• Page Limits: The page limit for submissions has been clarified and corrected to 35 pages. Copies of professional licenses will not be included in this page count.
• Resumes: Resumes for initial IDIQ evaluation are sufficient. Task orders may require different resumes. Additional personnel resumes beyond key positions are permitted but will not be evaluated as extensively as key personnel.
• Professional Qualifications: Proof of licensure is required if a license number is not provided in Section E of the SF330. Copies of licenses are excluded from the page count.

• Construction Phase Services (CPS): CPS experience should be mentioned in Section F if it was part of those projects. Section H allows for detailed narrative descriptions of CPS experience, which may or may not be from Section F projects.
• Geographic Location: This criterion is not evaluated at the IDIQ level but is reserved for task order evaluations. No submission is required for this at the IDIQ stage.

Questions & Answers / Clarifications

Statement of Work (SOW) / Performance Work Statement (PWS)

This Statement of Work (SOW) outlines the requirements for Architect-Engineer (A-E) services for nationwide projects across five regions, encompassing all 18 Veteran Integrated Service Networks (VISNs). These services are of an architect or engineering nature, as defined by state law, and are required for maintenance, repair, alteration, and new construction projects for institutional, commercial, and industrial facilities.

• Procurement support

• Construction period services (submittal reviews, commissioning, project management, construction management)

• Multi-disciplinary planning and design

• Specific services such as IT Infrastructure, Mechanical, Electrical, Physical Security, Environmental Engineering, Fire Protection, Geotechnical, NEPA, Section 106 Compliance, Site Development, Surveying, Water and Wastewater Systems, Civil, Structural Engineering, and Commissioning.

• Development of Design Programs, Schematic Design, Design Development, Construction Documents, Peer Review, Specialty Consulting, Constructability Review, QA/QC, Construction Submittal Review, RFP for Design-Build, Facility Condition Assessments, Electrical and Seismic Studies, Value Engineering, Commercial and Medical Facilities design, space planning, furniture design, signage, and wayfinding.

• Security information and directives (VHA Directive 6500) must be followed.

• Affirmative procurement requirements for recycled-content and bio-based products are promoted.

• Energy and water efficiency, renewable energy, and waste minimization are key considerations.

• Ozone-depleting substances are restricted.

• Stormwater, soil, and erosion control measures must be included.

• Environmental permitting evaluations are required.

• Nationwide, encompassing all 50 States, Virgin Islands, Puerto Rico, Guam, American Samoa, and the Philippines.
