Captus 4000e Thyroid Uptake System

Statement of Need

Procurement, delivery, installation, calibration, and training for one (1) new Captus 4000e Thyroid Uptake System (P/N: 5430-30151) and one (1) compatible Neck Phantom (P/N: 5230-0038) for the Nuclear Medicine Clinic at Madigan Army Medical Center (MAMC).

• Procurement: Acquisition of a new, commercial-off-the-shelf Captus 4000e Thyroid Uptake System, a Thyroid Uptake Neck Phantom, and proprietary operational software.

• Delivery & Installation: Includes shipping, handling, logistics, and on-site installation at Madigan Army Medical Center.

• Calibration & Training: System calibration and virtual clinical application training for Nuclear Medicine staff.

• Captus 4000e Thyroid Uptake System (P/N: 5430-30151): Must include a 1024-channel MCA board, 2-inch NaI(Tl) detector, 20-inch All-in-One Touch Screen Computer (secure Windows environment), mobile stand with articulating arm, and software suite (thyroid uptake protocols, wipe test modules, bioassay monitoring, QA reporting).

• Thyroid Uptake Neck Phantom (P/N: 5230-0038): Made of clear Lucite, designed to represent a standard patient's neck, with a two-part insert and etched markings for precise alignment.

• Documentation: Electronic and physical operations manuals, quick-start guides, warranty documents.

• Conformance: Items must conform to provided industry standards. Manufacturer's technical documentation verifying conformance is required with proposals.

• All deliverables, including delivery and installation, must be completed on or before September 30, 2026.

• Terms: FOB Destination.

• Address: Madigan Army Medical Center, Attn: Clinical Investigations – Nicholas Ieronimakis, 9040 Jackson Ave, Tacoma, WA 98431.

• Access: Delivery carrier and technician must comply with Joint Base Lewis-McChord (JBLM) access control and security protocols.

• Condition: Equipment must be brand new in original OEM packaging; used or refurbished equipment is prohibited.

• Minimum twelve (12) month comprehensive OEM warranty covering parts, labor, on-site repairs, and software updates.

• Contractor must be an Authorized Reseller, guaranteeing full manufacturer technical support.

• This is a Statement of Need for a specific medical system critical for diagnostic thyroid uptake measurements, therapeutic dosage calculations, and regulatory compliance with NRC and DHA safety regulations.

Amendment / Modification

This document is an addendum to FAR 52.212-2, "EVALUATION—COMMERCIAL PRODUCTS AND COMMERCIAL SERVICES." It inserts a new paragraph (d) detailing the evaluation factors for award.

• FAR 52.212-2, paragraph (d) - Evaluation Factors for Award.

• Award will be made using Lowest Price Technically Acceptable (LPTA) procedures.

• The Government intends to evaluate quotes and award without discussions, but reserves the right to conduct exchanges if determined necessary.

• Evaluation Factors:

  • Factor 1 – Technical Acceptability: The government will evaluate if the quoted product meets the requirements in Attachment 1 and any applicable brand-name-or-equal salient characteristics. Quotes will be rated Acceptable/Unacceptable.
  • Factor 2 – Price: The government will evaluate the total quoted price for completeness and reasonableness. Incomplete pricing may make a quote ineligible for award. The government may use various price analysis techniques, including comparison of quoted prices against solicitation responses, independent government estimates, historical information, or market research for similar items. Prices must be determined fair and reasonable.
  • Responsibility: The government may review SAM, SPRS, FAPIIS, exclusions, and other available information to determine contractor responsibility. Contractors will be rated Responsible/Not Responsible.

• Basis for Award: The award will be made to the responsible quoter whose quote conforms to the solicitation, is technically acceptable, and has the lowest total evaluated price. No tradeoffs will be conducted, and non-price factors will not be comparatively evaluated.

Amendment / Modification

This document is an addendum to FAR 52.212-1 Instructions to Offerors for commercial products and commercial services. It details the required components of a quote submission.

• Volume I: Technical / Product Compliance: Requires manufacturer name, product name, part/model number, product literature/cut sheets, confirmation of compliance with Attachment 1, and OEM authorization letter if applicable. Page limit is 2 pages, excluding product literature.

• Volume II: Price: Requires a completed CLIN pricing schedule with firm-fixed prices for all items, including shipping, handling, or installation costs. If the price is lower than market price, Offerors must submit a letter confirming the reason (e.g., OEM letter, overstock, additional discount).

• Volume III: Representations: Requires active SAM registration and completed FAR 52.212-3 representations if not current in SAM. No page limit.

• Quotes must be submitted electronically by the stated date and time; late quotes will not be considered.

• Offerors must provide sufficient product information for technical acceptability. Simply stating "will comply" is insufficient.

• Quoters must be an OEM, authorized dealer, distributor, reseller, or authorized service provider, verified by an OEM authorization letter or other document. OEM warranty and service must be provided and maintained by the OEM.

• All software licensing, warranty, and service must be in accordance with OEM terms and conditions.

Solicitation / RFP / RFQ

This document is a combined synopsis/solicitation for commercial products, specifically the Captus 4000e Thyroid Uptake System. It is issued as a Request for Quote (RFQ) and serves as the only solicitation, meaning a separate written solicitation will not be issued. The government is procuring this system in accordance with Attachment 1, Statement of Need / Salient Characteristics.

• The solicitation requires the contractor to provide the Captus 4000e Thyroid Uptake System.

• It also includes a "Neck Phantom for Thyroid System" and "Training" as separate line items.

• Quoters offering an "equal" product must provide documentation to demonstrate it meets or exceeds the salient characteristics.

• If the quoter is not the Original Equipment Manufacturer (OEM), an OEM authorization letter or similar documentation is required.

• Unique item identification and valuation are required for delivered items.

• The solicitation issue date is June 18, 2026.

• The offer due date is June 25, 2026, at 11:00 AM.

• The North American Industry Classification System (NAICS) code is 334510, with a size standard of 1250.

• Delivery is FOB Destination, with a delivery date on or before September 30, 2026, for all items.

• The contract administration will be handled by the Defense Health Agency.

• Submit quotes to denise.d.bond.civ@health.mil.

• Offers must be signed and dated and include the solicitation number, offeror's name, address, telephone number, Unique Entity Identifier (UEI), and EFT indicator.

• The government will evaluate offers based on Technical / Product Compliance and Price.

• The government intends to award a contract to the responsible offeror whose offer is most advantageous, price and other factors considered.

• Offerors must hold prices firm for 60 calendar days from the offer due date.

• This acquisition is set aside for Small Business.

• The solicitation incorporates FAR clauses by reference, including those related to commercial products and services, small business programs, and payment instructions.

• Attachment 1 (Statement of Work), Attachment A (Addendum to FAR 52.212-1), and Attachment B (Addendum to FAR 52.212-2) are referenced and should be reviewed for full requirements.
