Statement of Work (SOW) / Performance Work Statement (PWS)

To provide medical services for the National Institute of Occupational Safety and Health (NIOSH) employees, including those on emergency response teams, to ensure their health and safety in various occupational settings.

• Provide medical monitoring and surveillance, monitored care, records and data management, and support for emergency preparedness teams.

• Offer occupational medical services, including but not limited to, respirator clearance exams, hearing tests, and Beryllium exposure testing.

• Services are required at NIOSH campuses in Cincinnati, OH; Morgantown, WV; Pittsburgh, PA; and Spokane, WA.

• The contractor must be located near facilities (within approximately 20 miles) or able to provide services within eight (8) hours of request.

• Services are to be coordinated with the Contracting Officer's Representative (COR).

• Payment will be based on actual services provided according to the Contract Pricing Schedule.

• Invoicing is to be against the CLIN during which services are ordered.

Solicitation / RFP / RFQ

This is a combined synopsis/solicitation for commercial items, issued as Request for Quote (RFQ) 75D301-26-Q-78995, using Simplified Acquisition Procedures. The Centers for Disease Control and Prevention (CDC) intends to award a firm-fixed-price contract for medical services as described in the attached Statement of Work (SOW).

• All work shall be performed in accordance with the attached Statement of Work (SOW) RFQ 75D301-26-Q-78995.

• Quotes must include Total Price for all CLINs and Capabilities in accordance with the SOW and Evaluation Factors.

• The SOW provides full line items and service descriptions.

• Contract type: Firm-Fixed-Price.

• The acquisition is set aside for total small business under NAICS code 621999 (All Other Miscellaneous Ambulatory Health Care Services) with an SBA size standard of $21M annual revenue.

• A Firm-Fixed Price purchase order will be issued.

• Evaluation will be based on Lowest Price Technically Acceptable (LPTA).

• Offers will be assessed for technical acceptance in providing the ability to perform required medical testing at the designated geographic location.

• Price will be assessed for fairness and reasonableness, providing best value to the Government.

• CDC is looking for discounts on open-market pricing.
