• Procurement of pharmaceuticals, specifically CIPROFLOXACIN 400 MG/200ML-D5W.

• Products must be shippable by the United States Postal Service (USPS) unless otherwise approved.

• All pharmaceutical products must include bar code labeling at the unit-of-use package level, conforming to GS1-128 or HIBCC standards.

• Products must have a therapeutic equivalence code of "A" by the FDA, unless all drugs in the family group are "B" rated.

• Expiration dates must be no less than 16-18 months from the date of delivery.

• Drug Supply Chain Security Act (DSCSA) compliance is required for prescription pharmaceuticals, including Transaction History (TH), Transaction Information (TI), and Transaction Statement (TS).

• The contract period includes a base period and four option years, with option years not to exceed a 3% yearly increase.

• FOB: Destination.

• Offers must be submitted by 3:00 PM CDT on April 21, 2026.

• Offers must include a signed SF 1449, a pricing schedule in excel format, a valid wholesale distributor license, and a completed TAA Certificate (Section E.3).

• Quotes must include all items to be considered compliant.

• Evaluation will be based on Delivery, Past Performance, Price, and Technical factors using a comparative evaluation process.

• The Government reserves the right to consider quotations other than the lowest price if they provide additional benefit.

• This acquisition is set aside for Small Business.
